Description
Overview of the medicine
Cefixime is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. Sanfix 100 mg Suspension is primarily formulated for pediatric use.
Uses & Indications
Dosage
Adults
Not typically used for adults in suspension form; adult dosage for Cefixime is usually 200-400 mg daily in tablet form. If suspension is used due to swallowing difficulties, adjust dose based on standard Cefixime adult guidelines.
Elderly
No specific dosage adjustment needed unless severe renal impairment. Dose adjustment based on creatinine clearance is recommended for elderly patients with impaired renal function.
Renal_impairment
For creatinine clearance 21-60 ml/min, dose should be 75% of standard dose. For creatinine clearance < 20 ml/min, dose should be 50% of standard dose (e.g., 200 mg once daily or 100 mg twice daily).
How to Take
Sanfix 100 mg Suspension should be taken orally, with or without food. Shake the bottle well before each use to ensure uniform suspension. Use a calibrated measuring device to ensure accurate dosing.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, preventing the final transpeptidation step of peptidoglycan synthesis.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 2-6 hours.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. The remainder is excreted via biliary-fecal route.
Half life
Plasma half-life is typically 3-4 hours, but can be prolonged in renal impairment.
Absorption
Approximately 40-50% absorbed orally. Food may delay absorption but does not significantly affect the extent of absorption.
Metabolism
Not extensively metabolized in the body; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- Patients with a history of anaphylactic reactions to penicillin should be treated with caution due to potential cross-reactivity.
Drug Interactions
Probenecid
Increases Cefixime plasma concentration by inhibiting its renal excretion, potentially leading to increased efficacy or toxicity.
Carbamazepine
Elevated carbamazepine levels have been reported with concomitant use of Cefixime. Monitoring of carbamazepine levels may be considered.
Oral Contraceptives
The efficacy of oral contraceptives may be reduced when co-administered with Cefixime; alternative contraception may be advised.
Anticoagulants (e.g., Warfarin)
Concomitant administration may increase prothrombin time (PT) with or without clinical bleeding. Close monitoring of INR and adjustment of anticoagulant dose are recommended.
Storage
Store the dry powder below 30°C, protected from light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C) and do not freeze. Discard any unused portion after the recommended period.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor before use.

Lab Monitoring
- Renal function tests (for prolonged therapy or in patients with pre-existing renal impairment)
- Complete blood count (CBC) (occasionally for prolonged therapy)
- Liver function tests (LFTs) (occasionally, if liver issues are suspected or for prolonged use)
Doctor Notes
- Emphasize to parents/guardians the importance of completing the full course of therapy to prevent treatment failure and antibiotic resistance.
- Advise about potential gastrointestinal side effects and instruct them to seek medical attention if severe diarrhea or allergic reactions occur.
- Educate on proper reconstitution, storage, and administration techniques for the suspension.
Patient Guidelines
- Complete the entire prescribed course of medication, even if symptoms improve, to prevent the recurrence of infection and development of antibiotic resistance.
- Do not save any unused medication for future infections.
- Shake the suspension well before each dose.
- Discard any unused reconstituted suspension after the recommended period (usually 7-14 days).
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Cefixime generally does not impair the ability to drive or operate machinery. However, if side effects like dizziness or headaches occur, avoid driving or operating heavy machinery until you feel well.
Lifestyle Advice
- Maintain good hygiene practices to prevent reinfection.
- Drink plenty of fluids to stay hydrated, especially if experiencing diarrhea.
- Avoid sharing medicine with others.
