Overview of the medicine
Sanfix 400 mg capsule contains Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including those of the respiratory tract, urinary tract, and middle ear.
Uses & Indications
Dosage
Adults
400 mg once daily or 200 mg twice daily. Duration of treatment depends on the infection, typically 7-14 days.
Elderly
No dosage adjustment needed unless severe renal impairment. In such cases, follow renal impairment guidelines.
Renal_impairment
For creatinine clearance (CrCl) 21-60 mL/min, 300 mg daily. For CrCl < 20 mL/min, 200 mg daily.
How to Take
Take the capsule orally with or without food. It can be taken with food to reduce potential stomach upset.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis. This leads to the lysis and death of bacterial cells.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved in 2-6 hours after oral administration.
Excretion
Approximately 50-60% of an absorbed dose is excreted unchanged in the urine over 24 hours. Some is also excreted via bile.
Half life
Average plasma half-life is 3-4 hours, which may be prolonged in renal impairment.
Absorption
Approximately 40-50% orally absorbed. Food may delay absorption but does not significantly alter the extent of absorption.
Metabolism
Not extensively metabolized. A small fraction may be metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reaction to penicillin (due to potential cross-reactivity).
Drug Interactions
Warfarin
May increase anticoagulant effect (INR), monitor INR closely.
Probenecid
Increases cefixime plasma concentrations by decreasing renal excretion.
Carbamazepine
Elevated carbamazepine levels have been reported with co-administration.
Oral Contraceptives
Reduced effectiveness of oral contraceptives has been reported (though generally low risk).
Storage
Store in a cool, dry place below 30°C, away from direct light and moisture. Keep out of reach of children.
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Management should be supportive.
