Overview of the medicine
Seclo 40 mg Capsule contains Omeprazole, a proton pump inhibitor used to reduce stomach acid production. It is prescribed for conditions like heartburn, acid reflux (GERD), stomach ulcers, and to treat or prevent damage to the esophagus.
Uses & Indications
Dosage
Adults
For duodenal ulcer: 20 mg once daily for 2-4 weeks. For gastric ulcer: 20 mg once daily for 4-8 weeks. For GERD: 20 mg once daily for 4-8 weeks, severe cases 40 mg once daily. For Zollinger-Ellison Syndrome: Initial dose 60 mg once daily, adjusted as needed.
Elderly
No specific dose adjustment is required, but caution is advised in elderly patients, especially those with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Swallow the capsule whole with a glass of water, usually in the morning before breakfast. Do not chew or crush the capsule.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase enzyme (the proton pump) in the parietal cells of the stomach. This action effectively blocks the final step in gastric acid secretion, significantly reducing stomach acid levels.

Onset
Acid secretion inhibition begins within 1 hour, maximum effect within 2 hours.
Excretion
Approximately 80% of metabolites are excreted in the urine and about 20% in the feces.
Half life
Plasma elimination half-life is approximately 0.5 to 1.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 0.5 to 3.5 hours. Bioavailability is about 30-40%, increasing to about 60% with repeated doses due to saturation of presystemic metabolism.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 (CYP) enzyme system, specifically CYP2C19 and CYP3A4.

Contraindications
- Hypersensitivity to Omeprazole, substituted benzimidazoles, or any component of the formulation
- Concomitant use with nelfinavir (an antiretroviral drug)
Drug Interactions
Digoxin
Increased absorption and plasma levels of digoxin.
Warfarin
Increased INR/prothrombin time; monitor closely.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of clopidogrel.
Methotrexate
Increased plasma levels of methotrexate.
Nelfinavir, Atazanavir
Significantly reduced plasma levels of these antiretrovirals.
