Description
Overview of the medicine
Seclo 40 mg Injection contains Omeprazole, a proton pump inhibitor, used to reduce stomach acid production. It is prescribed for conditions like peptic ulcers, GERD, and Zollinger-Ellison syndrome, especially when oral administration is not feasible.
Uses & Indications
Dosage
Adults
The usual dose is 40 mg once daily for most indications, given as an intravenous infusion. For Zollinger-Ellison syndrome, the initial dose is 60 mg daily, adjusted according to patient response. Infusion typically over 20-30 minutes.
Elderly
No specific dose adjustment is required for elderly patients unless they have severe hepatic impairment. Administer with caution.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Administer Seclo 40 mg Injection intravenously as an infusion over 20-30 minutes. It should be reconstituted with the provided solvent or compatible solution (e.g., 100 mL of 0.9% NaCl or 5% Dextrose) and used within a specific time frame.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the gastric parietal cells. This action effectively blocks the final step in acid production, leading to a significant reduction in gastric acid secretion.
Pharmacokinetics
Onset
Within 30-60 minutes.
Excretion
Approximately 77% excreted in urine and 18% in feces, mainly as metabolites.
Half life
Plasma half-life is approximately 0.5 to 1 hour, but the duration of acid suppression is much longer.
Absorption
Rapidly absorbed after intravenous administration. Bioavailability is 100% via IV route.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to Omeprazole or any other substituted benzimidazoles.
- Concomitant use with Nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time have been reported, requiring monitoring.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19. Concomitant use should be avoided or monitored.
Methotrexate
Concomitant use may increase serum levels of methotrexate and its metabolite, potentially leading to methotrexate toxicity. Monitor carefully.
Diazepam and Phenytoin
Omeprazole may prolong the elimination of diazepam and phenytoin, necessitating dose adjustment.
Storage
Store unopened vials below 30°C. Protect from light and moisture. Do not freeze. Reconstituted solution should be used immediately or stored as per specific instructions (e.g., up to 6 hours at room temperature).
Overdose
There is no specific antidote for omeprazole overdose. Treatment should be symptomatic and supportive. Since omeprazole is highly protein-bound, dialysis is not likely to be effective.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Omeprazole is excreted in human breast milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.

Lab Monitoring
- Monitor liver function tests, especially in patients with pre-existing hepatic impairment.
- Periodically monitor serum magnesium levels in patients on prolonged treatment (more than 3 months) or concomitant with other drugs that can cause hypomagnesemia.
Doctor Notes
- Consider the lowest effective dose for the shortest duration necessary for all PPI therapy.
- Educate patients on potential long-term risks, especially bone fracture and C. difficile infection.
- Regularly assess the need for continued PPI therapy; consider step-down or discontinuation when appropriate.
- Be aware of drug interactions, particularly with clopidogrel and methotrexate.
Patient Guidelines
- Ensure proper administration by a healthcare professional.
- Inform your doctor about all other medications you are taking.
- Report any unusual or severe side effects immediately.
- Do not stop the medication suddenly without consulting your doctor.
Missed Dose Advice
If a dose is missed, administer it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Omeprazole may cause side effects like dizziness or visual disturbances (e.g., blurred vision) in some individuals. Patients should be advised to exercise caution when driving or operating machinery until they know how the medicine affects them.
Lifestyle Advice
- Avoid trigger foods and beverages that worsen acid reflux (e.g., spicy foods, caffeine, alcohol).
- Maintain a healthy weight and avoid large meals before bedtime.
- Elevate the head of your bed if you experience nighttime reflux symptoms.
- Avoid smoking, as it can worsen GERD symptoms.
