Description
Overview of the medicine
Segorin 500 mg Tablet contains Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria. It is effective against various Gram-positive and Gram-negative bacteria.
Uses & Indications
Dosage
Adults
The usual adult dosage of Cefixime is 200 mg to 400 mg daily, given as a single dose or in two divided doses. For Segorin 500 mg Tablet, it may be prescribed as one tablet once daily or as directed by the physician for specific severe infections.
Elderly
No dosage adjustment is generally required in elderly patients with normal renal function. However, renal function should be assessed, and dosage adjusted if necessary.
Renal_impairment
For patients with creatinine clearance < 60 mL/min, the dose should be reduced. For severe impairment (CrCl 21-60 mL/min), reduce to 300 mg daily. For CrCl < 20 mL/min, reduce to 200 mg daily.
How to Take
Segorin 500 mg Tablet should be taken orally with water, with or without food. It is recommended to take it at the same time each day to maintain consistent drug levels in the body. Complete the full course of treatment as prescribed by the doctor, even if symptoms improve.
Mechanism of Action
Cefixime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains essential for bacterial cell wall strength and rigidity, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Within 1-2 hours (peak plasma concentration).
Excretion
Primarily excreted unchanged in the urine (about 50-60% of the absorbed dose) and also via biliary excretion.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food can delay absorption but does not significantly affect the extent of absorption.
Metabolism
Not significantly metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or other cephalosporins
- Hypersensitivity to penicillins (due to potential cross-reactivity)
Drug Interactions
Probenecid
Increases Cefixime plasma concentrations and prolongs its half-life by reducing renal excretion.
Carbamazepine
Elevated carbamazepine levels reported when co-administered with Cefixime.
Warfarin and other anticoagulants
Increased prothrombin time (PT) with or without bleeding. Monitor INR/PT closely.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Gastric lavage may be indicated; otherwise, no specific antidote exists. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treat symptomatically and provide supportive care.
Pregnancy & Lactation
Pregnancy Category B. Cefixime has not been shown to cause harm in animal studies, but adequate human studies are lacking. Use during pregnancy only if clearly needed. Excreted in very low concentrations in breast milk; caution is advised when administering to a nursing mother.
