Overview of the medicine
Selex (Cefuroxime Axetil) is a broad-spectrum cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Most infections: 250-500 mg twice daily. Uncomplicated urinary tract infections: 125-250 mg twice daily. Gonorrhoea: 1000 mg as a single dose.
Elderly
Similar to adult dosage, but dose adjustment may be necessary in case of renal impairment.
Renal_impairment
Dosage should be reduced based on creatinine clearance (e.g., 125-250 mg once daily for severe impairment).
How to Take
Take orally with or after food for optimum absorption.
Mechanism of Action
Cefuroxime works by inhibiting bacterial cell wall synthesis, leading to bactericidal action against susceptible organisms.
Pharmacokinetics
Onset
Rapid absorption with peak plasma concentrations occurring 2-3 hours after an oral dose.
Excretion
Excreted unchanged in urine, primarily via glomerular filtration and tubular secretion.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Cefuroxime axetil is absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefuroxime. Oral bioavailability is approximately 37-52% when taken with food.
Metabolism
Not significantly metabolized in humans. The axetil moiety is hydrolyzed in the intestinal mucosa and blood.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime, any other cephalosporin antibiotic, or any of the excipients.
- History of severe hypersensitivity to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems).
Drug Interactions
Probenecid
Concomitant administration with probenecid increases the area under the serum concentration versus time curve by 50%.
Antacids/H2 blockers/PPIs
Drugs that reduce gastric acidity may decrease the bioavailability of cefuroxime axetil. It is recommended to take cefuroxime axetil at least 2 hours before or after antacids, H2 blockers, or proton pump inhibitors.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose of cephalosporins can cause cerebral irritation leading to convulsions. Management is primarily supportive; hemodialysis can be used to reduce serum cefuroxime concentrations.
Pregnancy & Lactation
Pregnancy Category B. Cefuroxime is excreted in low concentrations in breast milk; generally considered safe during lactation, but caution is advised.
