Description
Overview of the medicine
Sergel capsule contains Esomeprazole, a proton pump inhibitor (PPI) used to reduce the amount of acid produced in the stomach.
Uses & Indications
Dosage
Adults
GORD: 20-40 mg once daily. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. H. pylori eradication: 20 mg twice daily with appropriate antibiotics.
Elderly
No specific dose adjustment is generally required, but caution is advised in patients with severe renal or hepatic impairment.
Renal_impairment
No dose adjustment necessary for patients with renal impairment.
How to Take
Swallow the capsule whole, at least 1 hour before a meal. Do not chew or crush the capsule.
Mechanism of Action
Esomeprazole selectively and irreversibly blocks the H+/K+ ATPase enzyme (proton pump) in the gastric parietal cells, thereby reducing gastric acid secretion.

Onset
Within 1 hour.
Excretion
Approximately 80% renally and 20% in faeces.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed, bioavailability is approximately 50-68%. Food intake delays and decreases absorption.
Metabolism
Extensively metabolized in the liver by CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel.
Methotrexate
May increase methotrexate levels, requiring dose adjustment.
Diazepam, Phenytoin, Citalopram
May increase plasma levels of these drugs.
Ketoconazole, Itraconazole, Erlotinib
Reduced absorption due to increased gastric pH.

Store below 30°C, protect from moisture and light.
Overdose
Experience with overdose is limited. Symptomatic and supportive treatment should be provided.
