Overview of the medicine
Sergel 20 mg Tablet contains Esomeprazole, a proton pump inhibitor that reduces the amount of acid produced in your stomach. It is used to treat conditions like heartburn, acid reflux, and stomach ulcers.
Uses & Indications
Dosage
Adults
For GERD: 20 mg or 40 mg once daily for 4-8 weeks. For H. pylori eradication: 20 mg twice daily for 10 days (in combination therapy). For Zollinger-Ellison Syndrome: 40 mg twice daily, adjusted as needed.
Elderly
No dosage adjustment is generally necessary for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Take one tablet orally, usually once daily, at least one hour before a meal. Swallow the tablet whole; do not chew or crush it. If you have difficulty swallowing, disperse the tablet in a glass of non-carbonated water and drink it within 30 minutes.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase (proton pump) in the parietal cells of the stomach, thereby blocking the final step of acid secretion. This leads to a profound and prolonged reduction of gastric acid secretion.
Pharmacokinetics
Onset
Acid secretion inhibition occurs within 1 hour.
Excretion
Approximately 80% of the dose is excreted as metabolites in the urine, and the remainder in the feces.
Half life
Plasma elimination half-life is about 1 to 1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 60% after a single 20 mg dose, increasing to about 80% after repeated daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir (an antiretroviral drug).
Drug Interactions
Warfarin
Increased INR and prothrombin time; monitor patients closely for bleeding.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of its activation by CYP2C19.
Methotrexate
Increased and prolonged plasma levels of methotrexate, potentially leading to toxicity. Monitor methotrexate levels.
Atazanavir/Nelfinavir
Reduced plasma levels of these antiretrovirals, potentially leading to loss of virologic response. Concomitant use is contraindicated for nelfinavir.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and self-limiting (e.g., gastrointestinal symptoms, headache). No specific antidote is known. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Esomeprazole is likely excreted in human milk; caution should be exercised when administered to a nursing woman.
