Description
Overview of the medicine
Sergel 40 mg Injection contains Esomeprazole, a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), erosive esophagitis, duodenal ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
40 mg once daily by intravenous infusion for 7-10 days, then transition to oral therapy. For Zollinger-Ellison syndrome, initial dose 80 mg daily, then individualized.
Elderly
No specific dose adjustment is generally required for elderly patients, but caution is advised in those with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Administer by intravenous infusion over a period of 10 to 30 minutes after reconstitution. Do not administer by intravenous bolus or intramuscular injection.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that works by irreversibly binding to the H+/K+-ATPase enzyme system (the proton pump) on the secretory surface of the gastric parietal cells. This inhibits the final step in acid production, thereby reducing gastric acid secretion.

Onset
Within 30 minutes to 1 hour after intravenous administration.
Excretion
Metabolites are primarily excreted in the urine (approximately 80%) and in the feces (approximately 20%).
Half life
Approximately 1 to 1.5 hours.
Absorption
Rapidly absorbed after intravenous administration, demonstrating high systemic bioavailability.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily by CYP2C19 and to a lesser extent by CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased systemic exposure to digoxin. Monitor digoxin levels.
Warfarin
Increased INR and prothrombin time have been reported with concomitant use. Monitor INR.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19. Avoid concomitant use if possible.
Methotrexate
May increase serum methotrexate levels, particularly with high doses. Consider temporary withdrawal of esomeprazole.
Atazanavir/Nelfinavir
Reduced plasma concentrations of these antiretrovirals, potentially leading to loss of therapeutic effect. Contraindicated with nelfinavir.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children. Reconstituted solution should be used within 6 hours.

Limited experience with overdose. Symptoms reported with oral overdose are largely reversible and include gastrointestinal disturbances and weakness. Treatment should be symptomatic and supportive.
