Description
Overview of the medicine
Cefuroxime Axetil is a broad-spectrum antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Most infections: 250 mg twice daily for 7-10 days. Severe infections or bronchitis: 500 mg twice daily for 7-10 days. Uncomplicated UTI: 125 mg twice daily for 7-10 days. Uncomplicated Gonorrhea: 1000 mg as a single dose.
Elderly
No specific dose adjustment needed unless severe renal impairment. Refer to renal impairment guidelines.
Renal_impairment
Creatinine clearance 30 mL/min or greater: No dose adjustment needed. Creatinine clearance <30 mL/min: Reduce dose or increase dosing interval. For example, 250 mg once daily for most infections.
How to Take
For oral administration. To optimize absorption, Cefuroxime Axetil should be taken with food. Do not crush or chew the capsule; swallow whole.
Mechanism of Action
Cefuroxime Axetil inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Typically, therapeutic plasma concentrations are achieved within 1-2 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine, approximately 50% by glomerular filtration and 50% by tubular secretion.
Half life
Approximately 1.2 to 1.5 hours in adults with normal renal function.
Absorption
Rapidly absorbed from the gastrointestinal tract and hydrolyzed to cefuroxime. Enhanced absorption when taken with food.
Metabolism
Cefuroxime is not significantly metabolized in the liver; it is hydrolyzed in the intestinal mucosa and blood to active cefuroxime.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, any other cephalosporin antibiotics, or any component of the formulation.
- History of severe hypersensitivity reaction to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems, monobactams).
Drug Interactions
Probenecid
Concomitant administration with probenecid increases the area under the serum concentration versus time curve (AUC) of cefuroxime by approximately 50%.
Oral Contraceptives
Cefuroxime may affect gut flora, leading to lower estrogen reabsorption and reduced efficacy of combined oral contraceptives. Patients should be advised to consider alternative or additional contraceptive methods.
Antacids/H2-receptor antagonists
Drugs that reduce gastric acidity may reduce the bioavailability of cefuroxime axetil. It is recommended to administer cefuroxime axetil at least 1-2 hours apart from these agents.
Aminoglycosides and potent diuretics
Concurrent administration with aminoglycosides or potent diuretics may increase the risk of nephrotoxicity.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms include cerebral irritation leading to convulsions. Management is supportive, and hemodialysis may be effective in removing cefuroxime from the body.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed. Cefuroxime is excreted in breast milk in small amounts; use with caution in nursing mothers.
