Description
Overview of the medicine
Silox 500 mg Capsule is a broad-spectrum antibiotic used to treat a wide range of bacterial infections. It works by preventing the growth of bacteria.
Uses & Indications
Dosage
Adults
The usual adult dose is 500 mg every 8 hours, or 750 mg every 12 hours, depending on the severity of the infection. For severe infections, the dose may be increased to 1000 mg every 8 hours.
Elderly
No specific dose adjustment is required for the elderly unless there is significant renal impairment. Monitor renal function.
Renal_impairment
For creatinine clearance 10-30 ml/min, 500 mg every 12 hours. For creatinine clearance <10 ml/min, 500 mg every 24 hours.
How to Take
Take the capsule orally with a full glass of water. It can be taken with or without food, but taking it with food may help reduce gastrointestinal upset. Complete the full course of medication as prescribed by your doctor, even if symptoms improve.
Mechanism of Action
Xylocillin Hydrochloride, a beta-lactam antibiotic, acts by inhibiting the synthesis of bacterial cell walls. It binds to penicillin-binding proteins (PBPs) located on the bacterial cell membrane, which are essential for peptidoglycan synthesis, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Therapeutic effects typically observed within 30-60 minutes after the first dose.
Excretion
Primarily excreted unchanged by the kidneys (approximately 60-70% within 6 hours) via glomerular filtration and tubular secretion.
Half life
Approximately 1.5 - 2 hours in healthy adults.
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations reached within 1-2 hours after oral administration.
Metabolism
Minimally metabolized in the liver (less than 10% of the dose).
Side Effects
Contraindications
- Known hypersensitivity to Xylocillin Hydrochloride, other beta-lactam antibiotics (e.g., penicillins, cephalosporins), or any component of the formulation.
- History of severe allergic reaction to any penicillin.
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, increasing the risk of bleeding. Close monitoring of INR is recommended.
Probenecid
Decreases the renal tubular secretion of Xylocillin Hydrochloride, resulting in increased and prolonged blood levels.
Methotrexate
May reduce methotrexate clearance, potentially increasing its toxicity.
Oral Contraceptives
May reduce the effectiveness of oral contraceptives. Additional contraceptive methods should be considered.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe nausea, vomiting, diarrhea, and in rare cases, neurological effects such as seizures. Treatment is supportive and symptomatic. Gastric lavage and activated charcoal may be considered if ingestion is recent. Hemodialysis may be used to remove the drug from circulation in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have shown no risk to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Xylocillin Hydrochloride is excreted in human milk; use with caution in nursing mothers.

- Liver function tests (ALT, AST) for patients with hepatic impairment or during prolonged therapy.
- Complete blood count (CBC) for prolonged therapy, especially if signs of bone marrow suppression occur.
Doctor Notes
- Assess patient for hypersensitivity to beta-lactam antibiotics before initiation.
- Consider microbiological culture and susceptibility testing where appropriate.
- Monitor renal function in patients with pre-existing impairment or during prolonged high-dose therapy.
- Advise patients on the importance of completing the full course to prevent resistance.
Patient Guidelines
- Take the entire course of medication as prescribed, even if you start to feel better.
- Do not share your medication with others.
