Overview of the medicine
Sinapol-500 mg tablet contains Paracetamol, a widely used medication for relieving mild to moderate pain and reducing fever.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg (1-2 tablets) every 4-6 hours as needed, not exceeding 4000 mg (8 tablets) in 24 hours.
Elderly
Similar to adult dosage, but caution advised in patients with impaired hepatic or renal function.
Renal_impairment
Dose adjustment or increased dosing interval may be necessary in severe renal impairment.
How to Take
Take orally with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Paracetamol (Acetaminophen) primarily acts by inhibiting prostaglandin synthesis in the central nervous system, leading to its analgesic and antipyretic effects.
Pharmacokinetics
Onset
Pain relief typically begins within 30-60 minutes.
Excretion
Excreted mainly by the kidneys as glucuronide and sulfate conjugates.
Half life
Plasma half-life ranges from 1 to 4 hours.
Absorption
Rapid and complete absorption from the gastrointestinal tract.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation pathways. A small amount is metabolized by CYP2E1 to a toxic intermediate, NAPQI, which is normally detoxified by glutathione.
Side Effects
Contraindications
- Hypersensitivity to paracetamol or any component of the formulation
- Severe hepatic impairment or active liver disease
Drug Interactions
Alcohol
Chronic alcohol consumption increases the risk of hepatotoxicity with paracetamol.
Warfarin
May enhance the anticoagulant effect of warfarin, leading to increased bleeding risk.
Phenytoin, Carbamazepine, Phenobarbital
May increase the metabolism of paracetamol, reducing its effectiveness and increasing risk of hepatotoxicity.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Overdose can cause severe, potentially fatal hepatotoxicity. Symptoms may include nausea, vomiting, abdominal pain, and liver failure. Management involves gastric lavage, activated charcoal, and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Generally considered safe at recommended doses during pregnancy and lactation, but always consult a doctor before use.
