Overview of the medicine
Sizodon 1 mg Tablet contains Risperidone, an atypical antipsychotic medication used to treat schizophrenia, bipolar disorder (manic or mixed episodes), and irritability associated with autistic disorder in children and adolescents.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 1-2 mg once daily, titrating to a target dose of 4-6 mg/day. Bipolar I Disorder: Initially 2-3 mg once daily, titrating to a target dose of 2-6 mg/day. Autistic Irritability: Starting dose 0.5 mg once daily, titrate gradually.
Elderly
Lower starting doses (e.g., 0.5 mg twice daily), gradual titration under close medical supervision, especially for dementia-related psychosis.
Renal_impairment
Lower starting doses (e.g., 0.5 mg twice daily) and slow titration. Maximum daily dose should not exceed 4 mg.
How to Take
Take Sizodon tablet orally, with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Risperidone is a selective monoaminergic antagonist with high affinity for serotonergic 5-HT2 and dopaminergic D2 receptors. It also binds to alpha1-adrenergic and, with lower affinity, to H1-histaminergic and alpha2-adrenergic receptors. Its antipsychotic activity is believed to be mediated through a combination of D2 and 5-HT2 antagonism.

Onset
Initial effects may be observed within days, with full therapeutic effects typically seen within several weeks.
Excretion
Primarily excreted via urine (70%) and feces (14%) as risperidone and its metabolites.
Half life
Risperidone: approximately 3 hours; 9-hydroxyrisperidone (active metabolite): approximately 20-24 hours.
Absorption
Well absorbed orally, reaching peak plasma concentrations within 1-2 hours for risperidone and 3-6 hours for its active metabolite, 9-hydroxyrisperidone.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 to 9-hydroxyrisperidone (paliperidone), which is equally active.
Side Effects
Contraindications

Drug Interactions
Antihypertensive agents
May enhance hypotensive effects.
Dopamine agonists (e.g., levodopa)
May antagonize the effects of dopamine agonists.

Increased sedation and central nervous system depression.
CYP2D6 inhibitors (e.g., fluoxetine, paroxetine)
May increase plasma concentrations of risperidone and its active metabolite.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms. Management should be supportive, including maintaining a clear airway, adequate oxygenation, and ventilation. Gastric lavage and activated charcoal may be considered. Continuous cardiovascular monitoring should be initiated.
Pregnancy & Lactation
Pregnancy: Use only if the potential benefit justifies the potential risk to the fetus. Lactation: Risperidone is excreted in breast milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
