Overview of the medicine
Sizodon 4 mg Tablet contains Risperidone, an atypical antipsychotic medication used to treat schizophrenia, symptoms of bipolar disorder (manic or mixed episodes), and irritability associated with autistic disorder in children and adolescents.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 2 mg/day, increased to 4-6 mg/day. Bipolar mania: Initially 2-3 mg/day, adjusted based on response (max 6 mg/day). Irritability associated with autistic disorder: Start 0.5 mg/day, titrate up to 0.5-3 mg/day.
Elderly
Lower initial dose (e.g., 0.5 mg twice daily), slower titration, monitor closely for adverse effects.
Renal_impairment
Initial dose of 0.5 mg twice daily. Dose should be slowly titrated with careful monitoring.
How to Take
Take orally, with or without food. The tablet should be swallowed whole with water and not crushed or chewed.
Mechanism of Action
Risperidone is a selective monoaminergic antagonist with high affinity for serotonergic 5-HT2 and dopaminergic D2 receptors. It also binds to alpha1-adrenergic receptors, and with lower affinity, to H1-histaminergic and alpha2-adrenergic receptors. It acts by blocking dopamine and serotonin receptors in the brain, thereby helping to restore the balance of certain natural substances.
Pharmacokinetics
Onset
Initial therapeutic effects can be seen within days, but full antipsychotic effects may take several weeks.
Excretion
Approximately 70% of the dose is excreted in the urine and 14% in the feces within 1 week.
Half life
Risperidone: approximately 3 hours. Its active metabolite, 9-hydroxyrisperidone: approximately 21 hours.
Absorption
Well absorbed orally, peak plasma concentrations reached within 1-2 hours for risperidone and 3-4 hours for 9-hydroxyrisperidone. Absolute bioavailability of an oral tablet is 70%.
Metabolism
Primarily hepatic, largely via CYP2D6 to 9-hydroxyrisperidone (active metabolite), and to a lesser extent via CYP3A4.
Side Effects
Contraindications
- •Known hypersensitivity to risperidone or any component of the formulation.
- •Dementia-related psychosis in elderly patients (increased risk of death, black box warning).
- •Severe central nervous system (CNS) depression.
Drug Interactions
Carbamazepine
May decrease plasma concentrations of risperidone and its active metabolite.
Fluoxetine/Paroxetine
May increase plasma concentrations of risperidone and its active metabolite.
Antihypertensive agents
May potentiate orthostatic hypotension.
CNS Depressants (e.g., alcohol, benzodiazepines)
Increased risk of CNS depression.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms. Management is supportive, including maintaining an open airway, ensuring adequate oxygenation and ventilation, and monitoring cardiovascular function. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as risperidone and its active metabolite are excreted in human milk.
