Description
Overview of the medicine
Somazole 20 mg capsule contains Esomeprazole, which is a proton pump inhibitor (PPI). It works by reducing the amount of acid produced in the stomach, thus helping to treat conditions such as heartburn, acid reflux, and ulcers.
Uses & Indications
Dosage
Adults
Typically 20 mg or 40 mg once daily, depending on the indication. For GERD, usually 20 mg daily for 4-8 weeks. For H. pylori eradication, 20 mg twice daily for 7-14 days.
Elderly
No specific dosage adjustment is generally required for elderly patients, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment is needed for patients with renal impairment.
How to Take
Take one capsule orally at least one hour before a meal, preferably in the morning. Swallow the capsule whole with water; do not chew, crush, or open it. If difficulty swallowing, the capsule can be opened and the contents mixed with a small amount of water or soft food and consumed immediately.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase enzyme system (the 'proton pump') in the gastric parietal cells. This inhibition prevents the final step of acid secretion, leading to a reduction in gastric acidity.
Pharmacokinetics
Onset
Acid inhibitory effect starts within 1 hour.
Excretion
Approximately 80% of metabolites are excreted in the urine and the remainder in the feces.
Half life
Plasma elimination half-life is approximately 1.5 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma levels typically occurring within 1-2 hours. Bioavailability is approximately 64% after a single 20 mg dose, increasing with repeated dosing.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 system (CYP2C19 and CYP3A4 enzymes).
Side Effects
Contraindications
- •Known hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir (an antiretroviral drug).
Drug Interactions
Digoxin
May increase the absorption and levels of digoxin.
Warfarin
May increase INR and prothrombin time, requiring monitoring.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel.
Methotrexate
May increase serum levels of methotrexate, potentially leading to toxicity.
Antifungal agents (e.g., ketoconazole)
Absorption may be reduced due to increased gastric pH.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms are generally mild and may include drowsiness, tachycardia, nausea, and abdominal pain. There is no specific antidote. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. Esomeprazole is likely excreted in human milk; caution should be exercised when administered to a nursing mother.
