Description
Overview of the medicine
Somazole 40 mg Capsule is a proton pump inhibitor (PPI) used to treat conditions where the stomach produces too much acid, such as gastroesophageal reflux disease (GERD), stomach ulcers, and Zollinger-Ellison syndrome. It works by reducing the amount of acid in the stomach.
Uses & Indications
Dosage
Adults
GERD/Erosive Esophagitis: 40 mg once daily for up to 8 weeks. Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted based on patient response.
Elderly
No specific dosage adjustment needed, but caution with comorbidities.
Renal_impairment
No dosage adjustment necessary.
How to Take
Take whole capsule at least 30-60 minutes before a meal, preferably in the morning. Do not chew, crush, or break the capsule.
Mechanism of Action
Pantoprazole irreversibly binds to the H+/K+-ATPase pump (proton pump) in the gastric parietal cells, inhibiting both basal and stimulated acid secretion. This leads to a sustained suppression of gastric acid production.

Mainly renal (approximately 71%) and fecal (approximately 18%).

Approximately 1 hour (terminal elimination half-life is longer in poor metabolizers)
Absorption
Well absorbed after oral administration; bioavailability is about 77%. Peak plasma concentrations achieved in 2-2.5 hours.
Metabolism
Extensively metabolized in the liver via cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications

Drug Interactions
Digoxin
Increased digoxin levels (monitor).
Warfarin
Increased INR and prothrombin time.
Methotrexate
Possible increased methotrexate levels, especially with high doses.
Ketoconazole, Itraconazole, Erlotinib
Decreased absorption due to increased gastric pH.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include drowsiness, tachycardia, nausea, headache, and abdominal pain. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in breast milk, so caution is advised during lactation.
