Description
Overview of the medicine
Somazole 40 mg Injection contains Pantoprazole, a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Usually 40 mg once daily by slow intravenous injection or infusion. In severe cases or Zollinger-Ellison syndrome, up to 80 mg twice daily may be given.
Elderly
No specific dose adjustment is generally required.
Renal_impairment
No dose adjustment is generally required for patients with impaired renal function.
How to Take
For intravenous use. Administer as a slow intravenous injection over 2-15 minutes or as an intravenous infusion over 15-30 minutes. The reconstituted solution must be used immediately.
Mechanism of Action
Pantoprazole irreversibly blocks the H+/K+-ATPase enzyme (the proton pump) in the gastric parietal cells, thereby inhibiting the final step in gastric acid secretion.
Pharmacokinetics
Onset
Within 15-30 minutes for significant acid suppression.
Excretion
Mainly excreted renally (approximately 80%) and the remainder in feces (20%).
Half life
Approximately 1 hour (but duration of action is much longer due to irreversible binding to the proton pump).
Absorption
Rapidly absorbed after intravenous administration, reaching peak plasma concentrations within minutes.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
- •Co-administration with atazanavir or nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time, increasing risk of bleeding.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased and prolonged serum levels of methotrexate.
Atazanavir/Nelfinavir
Reduced plasma concentrations of these antivirals, leading to potential loss of therapeutic effect.
Iron Salts/Ketoconazole/Erlotinib
Decreased absorption of drugs whose absorption is pH-dependent.
Storage
Store below 25-30°C, protected from light and moisture. Do not freeze. Reconstituted solution should be used immediately.
Overdose
There is no known specific antidote. In case of overdose, symptomatic and supportive treatment should be given.
Pregnancy & Lactation
Use in pregnancy only if clearly needed and the potential benefit outweighs the potential risk to the fetus. Caution should be exercised when administered to a nursing mother as pantoprazole is excreted in human milk.
