Description
Overview of the medicine
Sonexim is a combination medicine containing Naproxen, a non-steroidal anti-inflammatory drug (NSAID), and Esomeprazole, a proton pump inhibitor (PPI). It is used to treat symptoms of osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis in patients who are at risk of developing NSAID-associated gastric ulcers.
Uses & Indications
Dosage
Adults
One Sonexim 500 mg/20 mg delayed-release tablet orally once or twice daily, depending on the severity of the condition and clinical response.
Elderly
Use with caution; lower doses may be considered. Monitor for adverse effects, especially gastrointestinal and renal.
Renal_impairment
Not recommended for patients with moderate to severe renal impairment (creatinine clearance < 30 mL/min). Use with caution if mild impairment.
How to Take
Take the delayed-release tablet whole with water at least 30 minutes before a meal. Do not crush, chew, or split the tablet.
Mechanism of Action
Naproxen inhibits prostaglandin synthesis by blocking cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing inflammation, pain, and fever. Esomeprazole inhibits the H+/K+-ATPase (proton pump) in gastric parietal cells, leading to a reduction in gastric acid secretion.

Onset
Naproxen: ~1 hour for analgesia; Esomeprazole: ~1 hour for acid suppression.
Excretion
Naproxen: Primarily urine (as metabolites). Esomeprazole: Primarily urine (as metabolites) and feces.
Half life
Naproxen: 12-17 hours; Esomeprazole: 1-1.5 hours.
Absorption
Both Naproxen and Esomeprazole are well absorbed after oral administration. Esomeprazole is formulated for delayed release to bypass gastric acid degradation.
Metabolism
Naproxen: Primarily hepatic via CYP450, then glucuronidation. Esomeprazole: Extensively hepatic via CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Known hypersensitivity to Naproxen, Esomeprazole, or any NSAID
- •History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- •In the setting of coronary artery bypass graft (CABG) surgery
- •Late pregnancy (third trimester)
Drug Interactions
Digoxin
Increased Digoxin levels.
Clopidogrel
Increased risk of bleeding.
SSRIs/SNRIs
Increased risk of GI bleeding.
Methotrexate
Increased Methotrexate toxicity.
CYP2C19 inhibitors/inducers
May alter Esomeprazole levels.
Diuretics and ACE Inhibitors
Reduced hypotensive effect and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of Naproxen overdose include lethargy, drowsiness, epigastric pain, nausea, vomiting, and GI bleeding. Esomeprazole overdose symptoms are generally mild, including drowsiness and GI disturbances. Management includes symptomatic and supportive care. Gastric lavage and activated charcoal may be considered. Hemodialysis is not useful.
Pregnancy & Lactation
Pregnancy: Avoid in the third trimester due to potential fetal cardiovascular effects. Use in first and second trimesters only if clearly needed and benefit outweighs risk. Lactation: Both Naproxen and Esomeprazole are excreted in breast milk. A decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
