Overview of the medicine
Ceftriaxone is a broad-spectrum, third-generation cephalosporin antibiotic used to treat a variety of bacterial infections. It is administered via injection.
Uses & Indications
Dosage
Adults
Usual adult dose is 1-2 gm administered once daily (or in two equally divided doses) depending on the severity of the infection. Max dose 4 gm/day.
Elderly
No specific dose adjustment is needed for elderly patients with normal renal and hepatic function. Refer to adult dosage.
Renal_impairment
For severe renal impairment (creatinine clearance <10 mL/min) without hemodialysis, the dose should not exceed 2 gm daily. No adjustment needed for mild to moderate impairment.
How to Take
Ceftriaxone can be administered by deep intramuscular injection or by intravenous infusion. For IV administration, it should be infused over 30 minutes. IM injections should be given into a large muscle mass.
Mechanism of Action
Ceftriaxone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bactericidal activity.

Onset
Peak plasma concentrations reached within 2-3 hours after IM injection; immediately after IV administration.
Excretion
Excreted through both renal and biliary routes (approximately 50-60% in urine, 40-50% in bile/feces).
Half life
Approximately 5.8-8.7 hours.

Rapid and complete absorption after intramuscular (IM) injection. 100% bioavailability after intravenous (IV) administration.
Metabolism
Limited hepatic metabolism; approximately 33-67% excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to ceftriaxone, any other cephalosporin, or any of the excipients.
- •Neonates (≤28 days) with hyperbilirubinemia, especially preterm neonates, due to the risk of bilirubin encephalopathy.
- •Concomitant intravenous calcium-containing solutions in neonates.
Drug Interactions
Oral Anticoagulants
May increase the anticoagulant effect and the risk of bleeding.
Calcium-containing solutions
Concomitant use in neonates is contraindicated due to precipitation risk. In other age groups, administer sequentially with adequate flush.
Loop Diuretics (e.g., Furosemide)
May increase the nephrotoxicity of cephalosporins, although less commonly reported with ceftriaxone.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children. Reconstituted solutions should be used immediately or within specified timeframes (e.g., 6 hours at room temperature, 24 hours in a refrigerator).
Overdose
In case of overdose, symptoms may include nausea, vomiting, and diarrhea. Management is primarily supportive and symptomatic. Hemodialysis or peritoneal dialysis is not effective in removing ceftriaxone from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. Ceftriaxone is excreted in low concentrations in breast milk; caution is advised when administered to a nursing mother.
