Overview of the medicine
Specbac 250 mg Injection is a broad-spectrum antibacterial agent used to treat a wide range of bacterial infections. It works by inhibiting bacterial cell wall synthesis, leading to bacterial death. It is administered intravenously or intramuscularly.
Uses & Indications
Dosage
Adults
250 mg - 1 g every 8 hours, depending on the severity of infection. Administered by deep intramuscular or slow intravenous injection over 3-5 minutes, or by intravenous infusion over 20-60 minutes.
Elderly
Dosage adjustment may be required in elderly patients, especially those with impaired renal function. Usually, lower doses are administered.
Renal_impairment
Dosage should be reduced in patients with significant renal impairment. Creatinine clearance monitoring is essential. For CrCl < 20 mL/min, administer half the usual dose.
How to Take
For intravenous (IV) or intramuscular (IM) administration. For IV, dissolve the powder in sterile water for injection and administer slowly. For IM, dissolve and inject deep into a large muscle mass.
Mechanism of Action
Specbac (Spectramycin) acts by selectively inhibiting the synthesis of bacterial cell walls. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains essential for cell wall strength and integrity. This disruption leads to osmotic lysis and bacterial death.

Onset
Within minutes for IV, 30-60 minutes for IM.
Excretion
Predominantly excreted via the kidneys (glomerular filtration and tubular secretion) as unchanged drug.
Half life
Approximately 1.5 - 2 hours in adults with normal renal function.
Absorption
Rapidly and completely absorbed following intramuscular administration; peak plasma concentrations achieved within 30-60 minutes. Intravenous administration provides immediate systemic availability.
Metabolism
Minimal hepatic metabolism; primarily excreted unchanged.
Side Effects
Contraindications

Drug Interactions
Probenecid
Increases spectramycin plasma levels and prolongs its half-life by reducing renal tubular secretion.
Aminoglycosides
Concurrent use may increase the risk of nephrotoxicity, especially in patients with pre-existing renal impairment.
Oral Anticoagulants
May potentiate the anticoagulant effect of warfarin, requiring close INR monitoring.
Storage
Store below 30°C in a dry place, away from light. Reconstituted solution stable for 6 hours at room temperature or 24 hours under refrigeration (2-8°C).
Overdose
Symptoms of overdose may include convulsions, encephalopathy, and neuromuscular hypersensitivity. Management is supportive, including discontinuing the drug and hemodialysis if severe renal impairment is present.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy; only if clearly needed. Spectramycin is excreted in breast milk in small amounts; use with caution during lactation. Consult a doctor.
