Starin 100 mg Capsule is a potent HMG-CoA reductase inhibitor used to reduce elevated cholesterol levels and prevent cardiovascular events.
Uses & Indications
Dosage
Adults
The usual starting dose is 50 mg once daily. The dose may be increased to 100 mg once daily, or up to a maximum of 200 mg once daily, based on patient response and tolerability. Administer preferably in the evening.
Elderly
No specific dose adjustment is required, but caution should be exercised. Start with the lowest effective dose.
Renal_impairment
For patients with moderate to severe renal impairment (CrCl < 30 mL/min), a starting dose of 50 mg once daily is recommended. Close monitoring is advised.
How to Take
Starin capsules are for oral administration. They can be taken with or without food. Swallow the capsule whole with water; do not chew or crush.
Mechanism of Action
Starin selectively inhibits HMG-CoA reductase, an enzyme crucial for cholesterol synthesis in the liver. This inhibition leads to a decrease in hepatic cholesterol production, which in turn stimulates the upregulation of LDL receptors, leading to increased clearance of LDL-C from the bloodstream.
Pharmacokinetics
Onset
Initial lipid-lowering effects observed within 1 week; maximal effect typically seen by 2-4 weeks.
Excretion
Mainly excreted in the feces (approximately 70%) and to a lesser extent in the urine (approximately 30%).
Half life
Elimination half-life is approximately 10-14 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations achieved within 1-2 hours. Bioavailability is approximately 80%.
Metabolism
Extensively metabolized in the liver, primarily via CYP3A4, to active and inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to the active substance or to any of the excipients.
- •Active liver disease or unexplained persistent elevations of serum transaminases.
- •Pregnancy and lactation.
- •Concomitant use with strong CYP3A4 inhibitors.
Drug Interactions
Warfarin
Potentiation of anticoagulant effect, requiring close monitoring of INR.
Gemfibrozil and other fibrates
Increased risk of myopathy/rhabdomyolysis.
CYP3A4 Inhibitors (e.g., clarithromycin, itraconazole, protease inhibitors)
Increased plasma concentrations of Starin, increasing risk of myopathy.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
There is no specific treatment for Starin overdose. In the event of an overdose, the patient should be treated symptomatically, and supportive measures instituted as required. Liver function tests and CK levels should be monitored.
Pregnancy & Lactation
Starin is contraindicated during pregnancy and breastfeeding. Women of childbearing potential should use effective contraception during treatment. It is unknown whether Starin is excreted in human milk.
