Description
Overview of the medicine
Sugatrol is an oral hypoglycemic agent containing Glimepiride, used to lower blood glucose levels in patients with type 2 diabetes mellitus when diet and exercise alone are not sufficient.
Uses & Indications
Dosage
Adults
Initial dose: 1-2 mg once daily. Maintenance dose: 1-4 mg once daily. Max dose: 8 mg once daily.
Elderly
Start with 1 mg once daily, titrate carefully. Avoid doses > 4 mg daily.
Renal_impairment
Initial dose 1 mg once daily, titrate carefully based on renal function and glycemic control. Max dose 4 mg daily if GFR > 22 mL/min. Not recommended for severe impairment.
How to Take
Taken orally, usually once daily with breakfast or the first main meal of the day.
Mechanism of Action
Glimepiride primarily lowers blood glucose by stimulating the release of insulin from pancreatic beta cells. It binds to the sulfonylurea receptor on the beta cell, leading to closure of ATP-sensitive potassium channels, membrane depolarization, and calcium influx, which triggers insulin secretion. It may also have some extrapancreatic effects.

Absorption

Metabolism
Completely metabolized by oxidative biotransformation (primarily by CYP2C9) to two main metabolites: a cyclohexylhydroxy-methyl derivative and a carboxyl derivative.

Contraindications
- Known hypersensitivity to glimepiride or other sulfonylureas or sulfonamides.
- Type 1 diabetes mellitus.
- Diabetic ketoacidosis, with or without coma.
- Severe renal or hepatic impairment.
- Pregnancy and lactation.

Alcohol
May potentiate or weaken hypoglycemic action.
Insulin and other oral antidiabetics
Increased hypoglycemic effect.
Corticosteroids, Diuretics, Estrogens, Thyroid hormones

NSAIDs, ACE inhibitors, MAO inhibitors, Beta-blockers, Sulfonamides
