Cefotaxime is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Usually 500 mg - 1 g IV/IM every 8-12 hours, up to 2 g every 4-8 hours for severe infections. Dosage should not exceed 12 g/day.
Elderly
Dosage adjustment may be required based on renal function.
Renal_impairment
Dosage reduction is necessary based on creatinine clearance.
How to Take
Administer intravenously (slowly over 3-5 minutes or as infusion over 20-60 minutes) or intramuscularly (deep IM injection). Reconstitute powder with sterile water for injection.
Mechanism of Action
Inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs).
Pharmacokinetics
Onset
Rapid (within minutes for IV administration).
Excretion
Mainly renal (via urine).
Half life
Approximately 1 to 1.5 hours.
Absorption
Rapidly absorbed after intramuscular (IM) or intravenous (IV) administration.
Metabolism
Primarily metabolized in the liver to desacetylcefotaxime (active metabolite).
Side Effects
Contraindications
- Hypersensitivity to cefotaxime, other cephalosporins, or any component of the formulation.
- History of severe hypersensitivity to penicillin (due to potential cross-allergenicity).
Drug Interactions
Probenecid
May increase cefotaxime plasma concentrations.
Aminoglycosides
Increased risk of nephrotoxicity; monitor renal function.
Oral anticoagulants
May potentiate the effects of oral anticoagulants (e.g., warfarin).
Storage
Store below 25-30°C. Protect from light. Reconstituted solution should be used immediately or within a specified time if refrigerated.
Overdose
Management is supportive and symptomatic. Hemodialysis may be useful in reducing serum cefotaxime concentrations.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.
