Description
Overview of the medicine
Telfast is an antihistamine used to relieve symptoms of seasonal allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives). It helps alleviate symptoms like sneezing, runny nose, itchy eyes/nose/throat, and skin rash/itching.
Uses & Indications
Dosage
Adults
For seasonal allergic rhinitis: 120 mg or 180 mg once daily. For chronic idiopathic urticaria: 180 mg once daily.
Elderly
No specific dose adjustment is generally required for elderly patients with normal renal function.
Renal_impairment
For adults and children 12 years and older with impaired renal function, a starting dose of 60 mg once daily is recommended.
How to Take
Telfast tablets should be taken orally with water, preferably before a meal. Do not take with fruit juices.
Mechanism of Action
Fexofenadine, the active ingredient in Telfast, is a selective peripheral H1-receptor antagonist. It blocks the action of histamine, a natural substance in the body that causes allergic symptoms, without significant sedation or anticholinergic effects.
Pharmacokinetics
Onset
Onset of action typically occurs within 1-3 hours.
Excretion
Primarily excreted unchanged in feces (approximately 80%) and urine (approximately 11%).
Half life
The elimination half-life is approximately 11-15 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached in approximately 1-3 hours.
Metabolism
Minimally metabolized; approximately 5% of the total dose is metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to fexofenadine or any components of the formulation.
Drug Interactions
Erythromycin and Ketoconazole
These drugs have been shown to increase fexofenadine plasma concentrations, though clinical significance is generally minimal.
Antacids (aluminum and magnesium hydroxide)
Concomitant administration of antacids containing aluminum and magnesium hydroxide within 15 minutes of Telfast significantly reduces fexofenadine bioavailability. It is recommended to separate administration by at least 2 hours.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, dizziness, and dry mouth. Management involves symptomatic and supportive treatment; there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Fexofenadine is excreted in human milk; caution should be exercised when administered to a nursing woman.
