Description
Overview of the medicine
Telfex 120 mg tablet contains Fexofenadine Hydrochloride, a non-sedating antihistamine used to relieve symptoms of seasonal allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives). It helps alleviate sneezing, runny nose, itchy watery eyes, and itchy skin rashes.
Uses & Indications
Dosage
Adults
For seasonal allergic rhinitis: 120 mg once daily. For chronic idiopathic urticaria: 180 mg once daily (use Telfex 180 mg if available).
Elderly
No dosage adjustment is generally required for elderly patients with normal renal function.
Renal_impairment
For adults with impaired renal function, the recommended starting dose is 60 mg once daily.
How to Take
Take the tablet orally with water, preferably before a meal. Do not take with fruit juices (e.g., grapefruit, orange, apple juice) as they may reduce absorption.
Mechanism of Action
Fexofenadine is a selective peripheral H1-receptor antagonist. It works by blocking the action of histamine, a natural substance in the body that causes allergic symptoms. By blocking H1 receptors, it prevents histamine from binding and causing symptoms like sneezing, itching, and watery eyes, without causing significant drowsiness.
Pharmacokinetics
Onset
Within 1 hour.
Excretion
Primarily excreted unchanged via feces (approximately 80%) and urine (approximately 11%).
Half life
Approximately 11-15 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached in 1-3 hours.
Metabolism
Minimally metabolized; approximately 5% of the total dose is metabolized.
Side Effects
Contraindications
- Known hypersensitivity to fexofenadine or any components of the tablet.
Drug Interactions
Erythromycin and Ketoconazole
Can increase plasma levels of fexofenadine, but this is generally not associated with increased adverse effects.
Antacids (aluminum and magnesium hydroxide containing)
May reduce the absorption of fexofenadine. Separate administration by about 2 hours.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, dizziness, and dry mouth. Management should be symptomatic and supportive, including measures to remove unabsorbed drug and standard supportive therapy.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Fexofenadine is excreted in human breast milk; caution should be exercised when administered to a nursing woman.
