Description
Overview of the medicine
Fexofenadine is a non-drowsy antihistamine used to relieve symptoms of seasonal allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives). It helps reduce sneezing, runny nose, itchy watery eyes, and skin itching.
Uses & Indications
Dosage
Adults
For Chronic Idiopathic Urticaria: 180 mg once daily. For Seasonal Allergic Rhinitis: 120 mg or 180 mg once daily.
Elderly
No dosage adjustment is recommended for elderly patients with normal renal function.
Renal_impairment
For patients with impaired renal function (creatinine clearance < 80 mL/min), the recommended initial dose is 60 mg once daily for chronic idiopathic urticaria and 60 mg once daily for seasonal allergic rhinitis. (180 mg is generally not recommended for initial use in severe impairment)
How to Take
Take the tablet orally with water. It can be taken with or without food. However, it should not be taken with fruit juices (e.g., grapefruit, orange, apple) as they can reduce the absorption of fexofenadine.
Mechanism of Action
Fexofenadine is a selective peripheral H1-receptor antagonist. It works by blocking the action of histamine, a natural substance in the body that causes allergic symptoms, without significantly crossing the blood-brain barrier, thereby reducing sedation.
Pharmacokinetics
Onset
Onset of action typically occurs within 1 hour.
Excretion
Approximately 80% is excreted unchanged in the feces via biliary excretion, and 10% in the urine.
Half life
The elimination half-life is approximately 11-15 hours.
Absorption
Rapidly absorbed after oral administration; peak plasma concentrations (Cmax) are reached approximately 1-3 hours after dosing.
Metabolism
Minimally metabolized; approximately 5% of the total dose is metabolized, primarily in the liver.
Side Effects
Contraindications
- Known hypersensitivity to fexofenadine or any of its excipients.
Drug Interactions
Erythromycin and Ketoconazole
Concomitant administration with erythromycin or ketoconazole has been shown to increase plasma levels of fexofenadine. However, no increase in adverse effects was observed.
Antacids (Aluminum/Magnesium-containing)
Administration of aluminum and magnesium containing antacids 15 minutes prior to fexofenadine can reduce fexofenadine absorption. Separate administration times.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, dizziness, and dry mouth. In the event of overdose, general symptomatic and supportive measures should be instituted. Hemodialysis does not effectively remove fexofenadine from blood.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether fexofenadine is excreted in human breast milk, so caution should be exercised when administered to a nursing mother.

- Routine laboratory monitoring is generally not required during fexofenadine therapy.
Doctor Notes
- Counsel patients on the importance of avoiding fruit juices (grapefruit, orange, apple) while taking fexofenadine to prevent reduced efficacy.
- Advise patients that while generally non-sedating, individual responses can vary, and caution should be exercised with activities requiring mental alertness.
