Description
Overview of the medicine
Telmicard is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to reduce the risk of cardiovascular events in patients aged 55 years or older at high risk.
Uses & Indications
Dosage
Adults
Usually 40 mg orally once daily. Dosage may be increased to a maximum of 80 mg once daily if needed.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with mild to moderate renal impairment. Caution is advised in severe renal impairment.
How to Take
Administer orally once daily, with or without food.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues, including vascular smooth muscle and adrenal gland. This blockage inhibits the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure.
Pharmacokinetics
Onset
Onset of antihypertensive effect within 3 hours; maximal reduction within 4-8 weeks.
Excretion
Almost entirely excreted in feces via bile; less than 1% excreted in urine.
Half life
Elimination half-life is approximately 24 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; absolute bioavailability is dose-dependent (approx. 42% for 40 mg, 58% for 160 mg). Food slightly reduces bioavailability.
Metabolism
Primarily metabolized via conjugation to form an inactive acylglucuronide.
Side Effects
Contraindications
- Pregnancy (especially 2nd and 3rd trimesters)
- Biliary obstructive disorders or severe hepatic impairment
- Hypersensitivity to telmisartan or any component of the formulation
- Concomitant use with aliskiren in patients with diabetes or renal impairment.
Drug Interactions
NSAIDs
May reduce the antihypertensive effect and increase risk of renal impairment.
Lithium
Increased serum lithium concentrations and toxicity.
Aliskiren
Increased risk of renal impairment, hypotension, and hyperkalemia.
Diuretics
May enhance the hypotensive effect.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia.

Store below 30°C (86°F) in a dry place, protected from light and moisture. Keep out of reach of children.

Symptoms of overdose include hypotension, dizziness, tachycardia, or bradycardia. Treatment is symptomatic and supportive. Telmisartan is not removed by hemodialysis.
Pregnancy & Lactation
Telmicard is contraindicated in the second and third trimesters of pregnancy due to fetal toxicity. It is not recommended during the first trimester. Due to the potential for adverse effects on the nursing infant, it is not recommended during lactation.
