Description
Overview of the medicine
Thyronor-100 mcg Tablet contains Levothyroxine Sodium, a synthetic thyroid hormone used to treat hypothyroidism (underactive thyroid) and certain types of thyroid cancer. It replaces the natural thyroid hormone that the body doesn't produce enough of.
Uses & Indications
Dosage
Adults
Initial dose typically 25-50 mcg once daily, adjusted in 4-6 week intervals based on TSH levels. Maintenance dose usually 100-200 mcg once daily.
Elderly
Lower initial dose (e.g., 12.5-25 mcg once daily) due to increased sensitivity and risk of cardiac events, with gradual titration.
Renal_impairment
No specific dosage adjustment needed, as levothyroxine is primarily metabolized and excreted extrarenally.
How to Take
Oral. Take once daily, preferably in the morning, on an empty stomach at least 30-60 minutes before breakfast and other medications. Tablets can be crushed for infants and children and mixed with a small amount of water.
Mechanism of Action
Levothyroxine acts as a synthetic form of thyroxine (T4), which is converted to its active metabolite liothyronine (T3) in the body. T3 and T4 bind to thyroid hormone receptors and exert metabolic effects, regulating metabolism, growth, and development.
Pharmacokinetics
Onset
Clinical effects typically begin within 3-5 days; full therapeutic effect may take several weeks.
Excretion
Mainly via bile and feces, with some renal excretion.
Half life
Approximately 6-7 days (longer in hypothyroidism, shorter in hyperthyroidism).
Absorption
Variable (40-80%) from the gastrointestinal tract, influenced by food and other medications. Best absorbed on an empty stomach.
Metabolism
Primarily deiodination in peripheral tissues (liver, kidneys, muscle) to T3 and reverse T3 (rT3).
Side Effects
Contraindications
- •Untreated subclinical or overt hyperthyroidism
- •Acute myocardial infarction
- •Acute myocarditis
- •Acute panhypopituitarism
- •Adrenal insufficiency (untreated)
Drug Interactions
Warfarin
Levothyroxine may enhance the anticoagulant effect of warfarin, requiring INR monitoring and possible dose reduction of warfarin.
Beta-blockers, Digitalis
Increased thyroid hormone levels may alter the effects of these drugs.
Proton Pump Inhibitors (PPIs)
May reduce gastric acidity, potentially impairing levothyroxine absorption.
Bile acid sequestrants (e.g., Cholestyramine, Colestipol)
May decrease levothyroxine absorption. Separate administration by 4-5 hours.
Antacids (aluminum and magnesium hydroxide), Calcium Carbonate, Iron preparations
May decrease levothyroxine absorption. Separate administration by 4 hours.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include signs of hyperthyroidism: palpitations, tachycardia, arrhythmias, tremors, anxiety, insomnia, sweating, heat intolerance, weight loss. Management involves reducing or discontinuing the dose, symptomatic treatment, and supportive care.
Pregnancy & Lactation
Levothyroxine is considered safe and essential during pregnancy. Thyroid hormone requirements usually increase during pregnancy. Regular monitoring of thyroid function and dose adjustment are crucial. It is excreted into breast milk in minimal amounts, generally considered safe for breastfeeding infants.
