Overview of the medicine
Thyronor 75 mcg tablet contains Levothyroxine Sodium, a synthetic thyroid hormone. It is used to treat hypothyroidism (underactive thyroid) and to suppress thyroid stimulating hormone (TSH) in patients with certain types of thyroid cancer or goiter.
Uses & Indications
Dosage
Adults
Initial dose typically 12.5-50 mcg daily, adjusted by 12.5-25 mcg increments every 4-6 weeks based on TSH levels. Maintenance dose 75-125 mcg daily. Taken once daily on an empty stomach, 30-60 minutes before breakfast.
Elderly
Lower initial doses (e.g., 12.5-25 mcg daily) are recommended, with careful titration due to increased risk of cardiac side effects.
Renal_impairment
No specific dose adjustment is typically required, but monitoring of thyroid function is essential.
How to Take
Take orally once daily on an empty stomach, 30-60 minutes before breakfast. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Levothyroxine acts as a replacement for endogenous thyroid hormones. It is deiodinated to liothyronine (T3) in peripheral tissues. T3 and T4 (thyroxine) then bind to thyroid hormone receptors in the cell nucleus, regulating gene expression and influencing basal metabolic rate, protein synthesis, and carbohydrate and lipid metabolism.

Contraindications

Drug Interactions
Tricyclic antidepressants

Oral anticoagulants (e.g., warfarin)

Amiodarone, Beta-blockers, Propylthiouracil
May alter thyroid hormone metabolism.
Antidiabetic agents (insulin, oral hypoglycemics)
May increase insulin or oral hypoglycemic requirements; monitor blood glucose.
Estrogens (oral contraceptives, hormone replacement therapy)

Antacids (aluminum & magnesium containing), sucralfate, iron supplements, calcium carbonate, cholestyramine, colestipol, sodium polystyrene sulfonate
May decrease absorption of levothyroxine; administer 4 hours apart.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include signs of hyperthyroidism: palpitations, tachycardia, arrhythmias, tremors, anxiety, insomnia, weight loss, increased appetite, heat intolerance, diarrhea, abdominal cramps. Treatment is symptomatic and supportive. Beta-blockers may be used for cardiac symptoms. Gastric lavage or activated charcoal may be considered if taken recently.
Pregnancy & Lactation
Pregnancy Category A. Levothyroxine is considered safe and essential during pregnancy. Thyroid hormone requirements usually increase during pregnancy. Dosage adjustments may be necessary. It is excreted into breast milk in minimal amounts and is considered compatible with breastfeeding at usual doses.
