Description
Overview of the medicine
Thyronor-125 mcg Tablet is a synthetic thyroid hormone (levothyroxine) used to treat an underactive thyroid gland (hypothyroidism) and certain types of thyroid cancer.
Uses & Indications
Dosage
Adults
Initial dose 25-50 mcg daily, adjusted by 12.5-25 mcg increments every 4-6 weeks based on TSH levels. Maintenance dose usually 1.6 mcg/kg/day.
Elderly
Lower initial doses (12.5-25 mcg daily) are recommended, with careful monitoring and slower titration due to increased sensitivity.
Renal_impairment
No dosage adjustment needed. Monitor thyroid function as usual.
How to Take
Take orally once daily on an empty stomach, preferably 30-60 minutes before breakfast, with a full glass of water. Do not take with food or other medications (especially calcium/iron supplements) for at least 4 hours.
Mechanism of Action
Levothyroxine sodium acts as a replacement for natural thyroid hormones (T4), which are normally produced by the thyroid gland. It helps to regulate the body's metabolism, energy levels, and growth.
Pharmacokinetics
Onset
Gradual; clinical effects take several weeks to manifest, and full therapeutic effect may take 4-6 weeks.
Excretion
Mainly renal (urine) and biliary/fecal excretion.
Half life
Approximately 6-7 days.
Absorption
Incompletely absorbed from the gastrointestinal tract (48-80%), absorption is increased by fasting.
Metabolism
Primarily hepatic deiodination to triiodothyronine (T3) and other inactive compounds; also conjugated.
Side Effects
Contraindications
- •Untreated adrenal insufficiency
- •Acute myocardial infarction
- •Acute thyrotoxicosis
- •Hypersensitivity to levothyroxine or any components of the formulation.
Drug Interactions
Warfarin
Levothyroxine may potentiate the anticoagulant effect of warfarin, requiring a reduction in warfarin dose.
Tricyclic antidepressants
Concurrent use may increase the toxicity of both drugs.
Bile acid sequestrants (e.g., cholestyramine)
May decrease levothyroxine absorption. Separate administration by at least 4 hours.
Antacids (e.g., aluminum, magnesium) and Iron/Calcium supplements
May decrease levothyroxine absorption. Separate administration by at least 4 hours.
Certain antiepileptic drugs (e.g., carbamazepine, phenytoin) and Rifampicin
May increase levothyroxine metabolism, requiring higher doses.
Storage
Store below 25°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are consistent with hyperthyroidism, including palpitations, nervousness, tremors, increased appetite, weight loss, heat intolerance, and insomnia. Treatment is symptomatic and supportive; beta-blockers may be used to manage symptoms.
Pregnancy & Lactation
Levothyroxine is generally safe and necessary during pregnancy and breastfeeding. Dosage requirements may change during pregnancy and should be monitored and adjusted by a physician.
