Uses & Indications
Dosage
Adults
Initial dose is typically 10 mg orally once daily. Dosage may be increased to 20 mg/day or up to 60 mg/day in divided doses, depending on patient response and tolerability.
Elderly
Start with a lower dose (e.g., 5 mg once daily) and adjust gradually, monitoring for adverse effects.
Renal_impairment
No specific dose adjustment is usually required for mild to moderate renal impairment, but caution is advised. For severe impairment, close monitoring is necessary.
How to Take
Timozin syrup should be administered orally, with or without food. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Timolol Maleate blocks both beta-1 and beta-2 adrenergic receptors. This results in decreased heart rate, reduced myocardial contractility, and lowering of blood pressure. In migraine prophylaxis, it is thought to stabilize vascular tone.

Drug Interactions
Digitalis glycosides
Increased risk of additive bradycardia.
Calcium channel blockers (e.g., verapamil, diltiazem)
Increased risk of bradycardia, hypotension, and atrioventricular conduction disturbances.
CYP2D6 inhibitors (e.g., quinidine, fluoxetine, paroxetine)
May increase plasma concentrations of timolol, enhancing its effects.
Catecholamine-depleting drugs (e.g., reserpine, guanethidine)
May have an additive effect, leading to excessive reduction of sympathetic activity, potentially causing hypotension and/or marked bradycardia.

Overdose
Symptoms of overdose include severe bradycardia, hypotension, bronchospasm, and acute cardiac failure. Treatment is symptomatic and supportive, including atropine for bradycardia, vasopressors for hypotension, and bronchodilators for bronchospasm.
Pregnancy & Lactation
Timolol should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, caution should be exercised when administered to a nursing mother.
