Description
Overview of the medicine
Timozin 50 mg Tablet contains Timolol Maleate, a non-selective beta-adrenergic receptor blocker used to treat various cardiovascular conditions like hypertension, angina pectoris, and for migraine prophylaxis. It also helps reduce the risk of re-infarction after myocardial infarction.
Uses & Indications
Dosage
Adults
For hypertension: Initially 10-20 mg once daily, adjusted based on response. Usual maintenance dose is 20-60 mg daily in single or divided doses. For 50mg, typically once daily. For angina, migraine prophylaxis, and post-MI: Usually 10-20 mg twice daily.
Elderly
Lower initial doses may be considered. Dosage adjustment may be necessary due to reduced renal and hepatic function.
Renal_impairment
Use with caution. Dose adjustment may be necessary in severe renal impairment (CrCl <15 mL/min) and in dialysis patients.
How to Take
Timozin 50 mg Tablet should be taken orally with water, with or without food. It is usually taken once or twice daily as directed by a physician. Do not crush or chew the tablet.
Mechanism of Action
Timolol is a non-selective beta-adrenergic receptor blocking agent. It competitively blocks both beta-1 and beta-2 adrenergic receptors. This results in decreased heart rate, reduced myocardial contractility, and lower blood pressure by reducing cardiac output and inhibiting renin release from the kidneys.
Pharmacokinetics
Onset
Onset of antihypertensive effect within 30-60 minutes.
Excretion
Mainly excreted in the urine as metabolites (approximately 20% as unchanged drug).
Half life
Plasma elimination half-life is approximately 4-6 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract after oral administration. Bioavailability is about 50% due to first-pass metabolism.
Metabolism
Extensively metabolized in the liver (primarily via CYP2D6) to inactive metabolites.
Side Effects
Contraindications
- •Sinus bradycardia
- •Second or third-degree atrioventricular block
- •Cardiogenic shock
- •Overt cardiac failure
- •Bronchial asthma or severe chronic obstructive pulmonary disease (COPD)
Drug Interactions
Digoxin
May enhance bradycardia.
Clonidine
Withdrawal of clonidine may result in rebound hypertension, especially if beta-blocker is also withdrawn abruptly.
Other beta-blockers
Concurrent use not recommended due to additive effects.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect of Timolol.
Calcium channel blockers (e.g., verapamil, diltiazem)
Increased risk of bradycardia and hypotension.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe bradycardia, hypotension, bronchospasm, and acute cardiac failure. Management involves supportive therapy, including atropine for bradycardia, vasopressors for hypotension, and bronchodilators for bronchospasm.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Excreted in breast milk; caution advised for nursing mothers. Consult your doctor.
