Description
Overview of the medicine
Tinidazole is an antimicrobial drug used to treat various parasitic infections such as trichomoniasis, giardiasis, amoebiasis, and bacterial vaginosis. It also has activity against anaerobic bacteria.
Uses & Indications
Dosage
Adults
For Trichomoniasis & Giardiasis: 2 g single oral dose or 1 g once daily for 3-5 days. For Amoebiasis: 2 g once daily for 2-3 days or 500 mg twice daily for 5-10 days. For Bacterial Vaginosis: 2 g single oral dose or 1 g once daily for 5 days.
Elderly
No specific dose adjustment is usually necessary for elderly patients unless there is significant renal or hepatic impairment.
Renal_impairment
Dosage reduction may be necessary in patients with severe renal impairment (creatinine clearance <20 mL/min). A single 2 g dose should be followed by 1 g every 48 hours.
How to Take
Take oral tablets with food to minimize gastrointestinal upset. Swallow the tablet whole; do not crush or chew. IV infusion should be administered slowly over 20-30 minutes.
Mechanism of Action
Tinidazole works by entering the microbial cell and getting reduced to its active form, which then binds to and damages microbial DNA, inhibiting nucleic acid synthesis and ultimately leading to cell death.

Onset
Onset of action typically within hours, depending on the infection type.
Excretion
Excreted by both renal (about 50%) and fecal routes (about 25%), with a small amount unchanged in urine.
Half life
Approximately 12-14 hours, allowing for once or twice daily dosing.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver by oxidation, hydroxylation, and conjugation.

Contraindications

Drug Interactions
Alcohol
Disulfiram-like reaction (abdominal cramps, nausea, vomiting, headaches, flushing)
Lithium
May increase serum lithium levels, leading to toxicity.
Cimetidine
May increase tinidazole levels by inhibiting its metabolism.
Phenytoin / Phenobarbital
May decrease tinidazole efficacy.
Oral Anticoagulants (e.g., Warfarin)
May potentiate the anticoagulant effect, increasing risk of bleeding. Monitor INR.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe gastrointestinal disturbances, ataxia, and peripheral neuropathy. Management is symptomatic and supportive; there is no specific antidote. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Category C in pregnancy (avoid in 1st trimester, use with caution in 2nd/3rd if clearly needed). Avoid during breastfeeding as it is excreted in breast milk.
