Description
Overview of the medicine
Tobirax Eye Drops contain Tobramycin, an aminoglycoside antibiotic, used to treat bacterial infections of the eye and its adnexa. It is effective against a broad spectrum of gram-negative and some gram-positive bacteria.
Uses & Indications
Dosage
Adults
1-2 drops into the affected eye(s) every 4 hours. For severe infections, 2 drops every hour until improvement, then reduce frequency.
Elderly
No special dosage adjustment required.
Renal_impairment
No dosage adjustment needed for topical ocular use due to minimal systemic absorption.
How to Take
For ophthalmic use only. Tilt head back, pull down the lower eyelid to form a pouch, instill the drops into the pouch without touching the eye with the dropper tip. Close eye gently for a few minutes. If using other eye medications, wait at least 5 minutes between applications.
Mechanism of Action
Tobramycin exerts its bactericidal effect by inhibiting bacterial protein synthesis. It irreversibly binds to the 30S ribosomal subunit, leading to misreading of mRNA and premature termination of protein synthesis.

Primarily renally excreted if systemically absorbed, but local elimination from ocular surface.

Side Effects
Contraindications
- •Hypersensitivity to Tobramycin or other aminoglycosides

Drug Interactions
Systemic aminoglycosides
Although systemic absorption is minimal, theoretical risk of increased systemic toxicity if used concurrently with other systemic aminoglycosides, neurotoxins, or ototoxic drugs.
Other ophthalmic medications
Allow at least 5 minutes between applications to avoid washout.
Storage
Store below 25°C in a cool, dry place, protected from light. Do not freeze.
Overdose
Due to minimal systemic absorption, an ocular overdose is unlikely to cause systemic toxicity. Ocular overdose may result in punctate keratitis, erythema, increased lacrimation, edema, and lid itching.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of fetal harm, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. It is not known whether tobramycin is excreted in human milk after topical ocular administration; however, systemic excretion is minimal.
