Description
Overview of the medicine
Tomycin (Tobramycin) is an aminoglycoside antibiotic primarily used to treat bacterial infections of the eye, such as bacterial conjunctivitis, keratitis, and blepharitis. It works by inhibiting bacterial protein synthesis, leading to bacterial cell death.
Uses & Indications
Dosage
Adults
For mild to moderate infections: Instill 1 or 2 drops into the affected eye(s) every 4-6 hours. For severe infections: Instill 2 drops into the affected eye(s) every hour until improvement, then reduce frequency.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
Not applicable for ophthalmic use. For systemic administration, dose adjustment is necessary in renal impairment.
How to Take
For eye drops, tilt your head back, pull down your lower eyelid, and instill the prescribed number of drops. Avoid touching the dropper tip to your eye or any other surface. Close your eye gently for a few minutes. If using ointment, apply a small amount (about 1 cm strip) to the inside of the lower eyelid.
Mechanism of Action
Tobramycin exerts its bactericidal effect by binding to the 30S ribosomal subunit of susceptible bacterial cells, thereby inhibiting bacterial protein synthesis and causing irreversible damage to the bacterial cell membrane.
Pharmacokinetics
Onset
Rapid onset of action for ophthalmic use (within minutes to hours).
Excretion
Excreted primarily unchanged via renal filtration in urine.
Half life
Systemic half-life is approximately 2-3 hours; not well-defined for ophthalmic use.
Absorption
Minimal systemic absorption occurs after topical ophthalmic administration. Systemic absorption can occur following intramuscular or intravenous administration.
Metabolism
Tobramycin is not significantly metabolized in the body.
Side Effects
Contraindications
- Hypersensitivity to Tobramycin or any other aminoglycoside antibiotics
- Known allergy to any component of the formulation
- Viral or fungal eye infections (unless co-administered with appropriate antiviral/antifungal therapy)
Drug Interactions
Other topical ophthalmic medications
If using more than one topical ophthalmic product, administer them at least 5-10 minutes apart.
Storage
Store at room temperature (below 30°C), away from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Ophthalmic overdose is unlikely to cause systemic toxicity due to minimal systemic absorption. If accidentally ingested, symptomatic and supportive treatment should be initiated.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when Tobramycin is administered to a nursing mother, as small amounts may be excreted in breast milk.
