Description
Overview of the medicine
Meropenem is a broad-spectrum carbapenem antibiotic used to treat a wide variety of bacterial infections. It is administered intravenously and is effective against both Gram-positive and Gram-negative bacteria, including many anaerobic strains.
Uses & Indications
Dosage
Adults
Typically 500 mg to 1 gm intravenously every 8 hours, depending on the severity and type of infection. For meningitis or severe infections, 2 gm every 8 hours.
Elderly
Dosage adjustment may be necessary based on renal function.
Renal_impairment
Dosage must be adjusted based on creatinine clearance. For CrCl 26-50 mL/min, 1 gm every 12 hours; for CrCl 10-25 mL/min, 500 mg every 12 hours; for CrCl <10 mL/min, 500 mg every 24 hours.
How to Take
Administer by intravenous bolus over 3-5 minutes or by intravenous infusion over 15-30 minutes.
Mechanism of Action
Meropenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This leads to the disruption of the cell wall structure, resulting in bacterial cell lysis and death.

Contraindications
- Hypersensitivity to meropenem or any other carbapenem antibiotics.
- Severe hypersensitivity (e.g., anaphylactic reaction, severe skin reaction) to any other beta-lactam antibacterial agent (e.g., penicillins, cephalosporins).
Drug Interactions
Probenecid
Competes with meropenem for active tubular secretion, resulting in increased plasma concentrations and prolonged half-life of meropenem.
Valproic Acid
Co-administration with meropenem may decrease valproic acid plasma concentrations, increasing the risk of seizures.
Oral Anticoagulants
Concomitant use with antibiotics, including meropenem, may enhance the anticoagulant effect of oral anticoagulants (e.g., warfarin).
Storage
Store below 30°C. Protect from light. Do not freeze. Reconstituted solution should be used immediately or within specified stability period.
Overdose
In case of overdose, discontinue meropenem and institute general supportive treatment. Meropenem can be removed by hemodialysis.
