Description
Overview of the medicine
Torped 250 mg Injection is a broad-spectrum antibiotic belonging to the fluoroquinolone class, used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
250 mg to 500 mg intravenously every 12-24 hours depending on the severity and type of infection. For specific infections, consult clinical guidelines.
Elderly
No specific dose adjustment typically required unless severe renal impairment is present. Monitor renal function.
Renal_impairment
Dose adjustment required based on creatinine clearance. For CrCl < 30 mL/min, reduce dose by half.
How to Take
For intravenous use only. Administer slowly over 60 minutes to minimize vein irritation and infusion-related reactions. Do not administer as a bolus injection.
Mechanism of Action
Inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Within 30-60 minutes after IV infusion.
Excretion
Mainly renal excretion, with a small portion excreted in feces.
Half life
Approximately 3-5 hours.
Absorption
Rapid and complete absorption after intravenous administration, 100% bioavailability.
Metabolism
Limited hepatic metabolism, primarily through CYP450 enzymes.
Side Effects
Contraindications
- Hypersensitivity to fluoroquinolones or any component of the injection
- History of tendon disorders related to fluoroquinolone use
- Children and adolescents (under 18 years) due to risk of cartilage damage
- Patients with epilepsy or other seizure disorders
Drug Interactions
NSAIDs
Concurrent use may increase the risk of CNS stimulation and convulsions.
Warfarin
May enhance anticoagulant effects, increasing bleeding risk.
Theophylline
May increase Theophylline levels, leading to toxicity.
Antacids (containing aluminum, magnesium, calcium)
May reduce absorption if given concurrently (though less relevant for IV).
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Reconstituted solution should be used immediately or stored for a limited time as per manufacturer's guidelines.
Overdose
In case of overdose, symptomatic treatment and supportive measures should be initiated. Adequate hydration is important. Hemodialysis or peritoneal dialysis may not be effective in removing the drug.
Pregnancy & Lactation
Use during pregnancy is generally not recommended unless the benefits outweigh the risks due to potential effects on cartilage development in the fetus. Excreted in breast milk; avoid breastfeeding or use with caution after consulting a physician.
