Description
Overview of the medicine
Tralin 25 mg Tablet contains Sertraline, a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder, obsessive-compulsive disorder, panic disorder, post-traumatic stress disorder, social anxiety disorder, and premenstrual dysphoric disorder.
Uses & Indications
Dosage
Adults
Initial dose for depression/OCD is usually 50 mg once daily. For panic disorder, PTSD, SAD, PMDD, initial dose is typically 25 mg once daily, increased to 50 mg after one week. The dose may be increased gradually up to a maximum of 200 mg daily based on response and tolerability.
Elderly
Similar to adult dosage, but caution advised. Start with a lower dose (e.g., 25 mg) and titrate slowly.
Renal_impairment
No dose adjustment is generally necessary for mild to moderate renal impairment. Caution advised in severe impairment due to lack of studies.
How to Take
Tralin tablets should be taken orally once daily, either in the morning or evening, with or without food. Consistent timing is recommended.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) by the presynaptic neuron, leading to an increased concentration of serotonin in the synaptic cleft. This action modulates mood, emotion, and behavior.
Pharmacokinetics
Onset
Therapeutic effects may take 2-4 weeks to become apparent, with full effect potentially taking longer.
Excretion
Mainly through feces (40-45%) and urine (13-16%) as metabolites.
Half life
Approximately 26 hours for sertraline, and 62-104 hours for its active metabolite, desmethylsertraline.
Absorption
Well absorbed from the gastrointestinal tract, peak plasma concentrations reached within 4.5 to 8.4 hours after once-daily dosing.
Metabolism
Extensively metabolized in the liver, primarily via N-demethylation to desmethylsertraline (inactive metabolite), and then by oxidative deamination and glucuronide conjugation.
Side Effects
Contraindications
- •Concomitant use with MAOIs (Monoamine Oxidase Inhibitors) or within 14 days of discontinuing MAOIs due to risk of serotonin syndrome.
- •Concomitant use with Pimozide.
- •Known hypersensitivity to sertraline or any component of the formulation.
Drug Interactions
Alcohol
Enhances the sedative effects of alcohol; concomitant use should be avoided.
Pimozide
Increased pimozide levels and cardiotoxicity; concomitant use is contraindicated.
Warfarin
Potentiation of anticoagulant effect, leading to increased bleeding risk.
MAOIs (Monoamine Oxidase Inhibitors)
Risk of serious, sometimes fatal, serotonin syndrome including hyperthermia, rigidity, myoclonus, autonomic instability, and mental status changes. Should not be used within 14 days of MAOIs.
Serotonergic drugs (e.g., Triptans, Tricyclic Antidepressants, Fentanyl, Lithium, Tramadol, St. John's Wort)
Increased risk of serotonin syndrome.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, vomiting, tachycardia, nausea, dizziness, agitation, and tremor. Seizures and ECG changes have been reported, rarely fatal. Treatment is primarily supportive and symptomatic. Gastric lavage, activated charcoal, and maintaining a clear airway are important. Monitor cardiac and vital signs.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Neonates exposed to SSRIs in late pregnancy have developed complications requiring prolonged hospitalization, respiratory support, and tube feeding. Sertraline is excreted in breast milk; caution should be exercised when administered to a nursing woman.
