Description
Overview of the medicine
Tralin 50 mg Tablet contains Sertraline, a selective serotonin reuptake inhibitor (SSRI) used to treat depression, anxiety disorders, panic disorder, obsessive-compulsive disorder (OCD), post-traumatic stress disorder (PTSD), and premenstrual dysphoric disorder (PMDD).
Uses & Indications
Dosage
Adults
Initial: 25-50 mg once daily. May increase by 25-50 mg weekly, up to a maximum of 200 mg daily. For depression and OCD, typical target dose is 50 mg/day. For panic, social anxiety, PTSD, PMDD, start with 25 mg/day.
Elderly
Similar to adult dosage, but titration should be done cautiously due to potential for increased sensitivity and slower clearance.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Administer orally, once daily, with or without food. It is often recommended to take it in the morning to avoid insomnia, but can be taken in the evening if preferred.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) by presynaptic neurons in the central nervous system, leading to an increase in serotonin concentration in the synaptic cleft. This potentiation of serotonergic neurotransmission is believed to be responsible for its antidepressant and anxiolytic effects.
Pharmacokinetics
Onset
Therapeutic effects may take 1-4 weeks to become apparent.
Excretion
Approximately 40-45% excreted in urine and 40-45% in feces, largely as metabolites.
Half life
Approximately 26 hours (range 22-36 hours) for sertraline; N-desmethylsertraline (active metabolite) has a longer half-life of 62-104 hours.
Absorption
Well absorbed orally, peak plasma concentration reached within 4.5-8.4 hours. Food slightly increases absorption.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19, CYP2B6, CYP2D6, CYP3A4, and CYP2C9, to an inactive N-desmethylsertraline metabolite (which has some weak activity).
Side Effects
Contraindications
- •Concomitant use with MAOIs (Monoamine Oxidase Inhibitors) or within 14 days of discontinuing an MAOI
- •Concomitant use with pimozide
- •Hypersensitivity to sertraline or any component of the formulation
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes). Contraindicated.
Alcohol
Enhances CNS depressant effects; avoid or limit.
Pimozide
Increased pimozide levels and cardiotoxicity. Contraindicated.
Triptans
Increased risk of serotonin syndrome; use with caution.
Warfarin
Increased prothrombin time (PT) / INR; monitor carefully.
NSAIDs/Aspirin
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, vomiting, tachycardia, tremor, agitation, dizziness, and rarely coma. Management involves supportive and symptomatic treatment, maintaining airway, oxygenation, and ventilation. Gastric lavage and activated charcoal may be considered. No specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. SSRIs used late in pregnancy may be associated with persistent pulmonary hypertension of the newborn (PPHN). Sertraline is excreted in breast milk; use with caution during lactation.
