Trazapin 15 mg Tablet contains Olanzapine, an atypical antipsychotic used to treat schizophrenia and bipolar disorder. It helps restore the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 10 mg/day, adjusted to 5-20 mg/day. Bipolar mania: Initially 10-15 mg/day, adjusted to 5-20 mg/day. Prevention of recurrence in bipolar disorder: 10 mg/day.
Elderly
Lower starting dose (e.g., 5 mg/day) and slower titration may be considered.
Renal_impairment
No specific dose adjustment is usually required, but caution is advised. Initiate with a lower dose (e.g., 5 mg/day).
How to Take
Take orally, once daily, with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Olanzapine acts as an antagonist at multiple receptor sites, including dopamine D1, D2, D3, D4, and D5 receptors, serotonin 5-HT2A, 5-HT2B, 5-HT2C, 5-HT3, and 5-HT6 receptors, alpha1-adrenergic receptors, and histamine H1 receptors. It also has antagonistic activity at muscarinic M1-M5 receptors.

Onset
Within a few days for symptom improvement, full effects may take weeks.
Excretion
Approximately 57% excreted in urine and 30% in feces, mainly as metabolites.
Half life
Approximately 21-54 hours (mean 33 hours).
Absorption
Well absorbed after oral administration, reaching peak plasma concentrations within 5-8 hours. Absorption is not affected by food.
Metabolism
Extensively metabolized in the liver, primarily by direct glucuronidation and by cytochrome P450 isozymes CYP1A2 and CYP2D6.
Side Effects
Contraindications

Drug Interactions
Fluvoxamine
May increase olanzapine levels; reduce olanzapine dose.
Carbamazepine
May decrease olanzapine levels; increase olanzapine dose.
Antihypertensives
Potentiated hypotensive effects.
Alcohol/CNS Depressants
Increased risk of sedation.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, slurred speech, rapid heartbeat, uncontrolled movements, and decreased consciousness. Treatment is supportive, focusing on maintaining airways and monitoring cardiovascular function.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. Not recommended during breastfeeding as it is excreted in breast milk.
