Description
Overview of the medicine
Trazodone is an antidepressant used to treat major depressive disorder. Due to its significant sedative properties, it is also frequently used off-label for the treatment of insomnia and anxiety.
Uses & Indications
Dosage
Adults
For depression: Initially 150 mg/day in divided doses. May be increased by 50 mg/day every 3-4 days to a maximum of 400 mg/day (outpatients) or 600 mg/day (inpatients). For insomnia: 25-100 mg at bedtime.
Elderly
Lower starting doses and slower titration are recommended due to increased sensitivity and potential for adverse effects. Initial dose of 75 mg/day in divided doses, may be increased cautiously.
Renal_impairment
No specific dosage adjustment is usually required, but caution is advised in patients with severe renal impairment.
How to Take
Take orally, preferably after a meal or light snack to reduce dizziness and improve absorption. Swallow the tablet whole with water.
Mechanism of Action
Trazodone acts as a serotonin antagonist and reuptake inhibitor (SARI). It selectively blocks serotonin 2A (5-HT2A) and alpha-1 adrenergic receptors, and to a lesser extent, inhibits serotonin reuptake. It also acts as an antagonist at histamine H1 receptors, contributing to its sedative effects.
Pharmacokinetics
Onset
Sedative effects can occur rapidly (within 30-60 minutes). Antidepressant effects may take 2-4 weeks to become noticeable.
Excretion
Approximately 75% of the dose is excreted in the urine within 72 hours, and about 20% is excreted in the feces.
Half life
Biphasic elimination with an initial half-life of 3-6 hours and a terminal half-life of 5-9 hours.
Absorption
Well absorbed orally, with peak plasma concentrations occurring in about 0.5-2 hours (fasted state) and 2-4 hours (with food). Bioavailability is increased with food.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 3A4 (CYP3A4) enzyme, to an active metabolite (m-chlorophenylpiperazine, m-CPP) and inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to trazodone or any excipients
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI
- •Acute recovery phase of myocardial infarction
Drug Interactions
Digoxin, Phenytoin
May increase plasma concentrations of digoxin and phenytoin. Monitor levels.
Monoamine Oxidase Inhibitors (MAOIs)
Increased risk of serotonin syndrome. Avoid concomitant use.
CYP3A4 Inducers (carbamazepine, rifampin)
Decreased trazodone plasma levels, potentially reducing efficacy. Consider dose increase of trazodone.
CNS Depressants (alcohol, benzodiazepines)
Enhanced CNS depression, drowsiness, and respiratory depression. Avoid concurrent use.
CYP3A4 Inhibitors (ketoconazole, ritonavir)
Increased trazodone plasma levels, potentially leading to increased adverse effects. Consider dose reduction of trazodone.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, vomiting, hypotension, priapism, respiratory arrest, and seizures. Management is supportive and symptomatic; gastric lavage or activated charcoal may be considered. Monitor cardiac function and vital signs.
Pregnancy & Lactation
Pregnancy Category C. Trazodone should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human breast milk; use with caution during lactation.
