Trazodone is an antidepressant used to treat major depressive disorder. It also has sedative properties and is often used off-label for insomnia. The 30 mg strength is typically used for lower doses or initial titration.
Uses & Indications
Dosage
Adults
For depression: Initial dose 50-150 mg/day in divided doses. May increase to 400 mg/day for outpatients, 600 mg/day for inpatients. For insomnia: 25-100 mg at bedtime.
Elderly
Start with lower doses, e.g., 25-50 mg/day, due to increased sensitivity and potential for orthostatic hypotension.
Renal_impairment
No specific dosage adjustment for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Take orally, usually after a meal or light snack to reduce dizziness and increase absorption. If used for insomnia, take at bedtime.
Mechanism of Action
Trazodone is a serotonin antagonist and reuptake inhibitor (SARI). It selectively inhibits serotonin reuptake and acts as an antagonist at 5-HT2A and alpha1-adrenergic receptors. These actions contribute to its antidepressant, anxiolytic, and sedative effects.

Sedative effects may appear within hours. Antidepressant effects typically take 2-4 weeks to become apparent.
Excretion
Approximately 75% excreted in urine, 25% in feces within 72 hours.
Half life
Biphasic elimination: initial half-life of 3-6 hours, terminal half-life of 5-9 hours.
Absorption
Well absorbed orally. Food significantly increases absorption (up to 2-fold) and decreases peak plasma concentrations, which may reduce side effects like dizziness.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, to an active metabolite (m-chlorophenylpiperazine, m-CPP) and other inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to trazodone or any component of the formulation.
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI.
- •Acute recovery phase of myocardial infarction.
Drug Interactions

Increased risk of serotonin syndrome.
Warfarin
Increased prothrombin time/INR.

Overdose
Symptoms include somnolence, dizziness, nausea, vomiting, priapism, respiratory arrest, and seizures. Management is supportive, including gastric lavage or activated charcoal if appropriate, and monitoring vital signs.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk. Not recommended during breastfeeding as trazodone is excreted into breast milk.
