Description
Overview of the medicine
Trevox (Levofloxacin) is a synthetic, broad-spectrum antibacterial agent of the fluoroquinolone class. It is used to treat various bacterial infections including respiratory tract, skin and soft tissue, urinary tract, and intra-abdominal infections.
Uses & Indications
Dosage
Adults
250 mg to 750 mg orally or intravenously once daily, depending on the type and severity of infection.
Elderly
No specific dose adjustment is required based solely on age, but renal function should be assessed and dose adjusted accordingly.
Renal_impairment
Dosage adjustment is necessary in patients with impaired renal function (creatinine clearance <50 mL/min) to prevent drug accumulation.
How to Take
Oral tablets can be taken with or without food. The intravenous solution should be administered slowly by infusion over a period of 60 to 90 minutes.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.
Pharmacokinetics
Onset
Onset of action within 1-2 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine, mainly via glomerular filtration and tubular secretion.
Half life
Approximately 6 to 8 hours.
Absorption
Rapid and nearly complete oral absorption, with peak plasma concentrations typically achieved within 1-2 hours. Oral bioavailability is approximately 99%.
Metabolism
Minimally metabolized in the liver; less than 5% of a dose is recovered in the urine as metabolites.
Side Effects
Contraindications
- Hypersensitivity to levofloxacin or other fluoroquinolones.
- History of tendon disorders related to fluoroquinolone administration.
- Epilepsy.
Drug Interactions
NSAIDs
Concomitant use with NSAIDs may increase the risk of CNS stimulation and convulsive seizures.
Warfarin
May enhance the anticoagulant effect of warfarin, increasing bleeding risk.
Corticosteroids
Increased risk of tendon rupture when co-administered.
Antacids (Mg/Al), Sucralfate, Iron, Zinc
May reduce oral absorption of levofloxacin. Administer levofloxacin at least 2 hours before or 2 hours after these agents.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of an overdose, symptomatic and supportive treatment should be initiated. ECG monitoring should be performed due to the potential for QT interval prolongation. Hemodialysis or peritoneal dialysis is not effective in removing levofloxacin from the body.
Pregnancy & Lactation
Pregnancy Category C. Levofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted into breast milk and therefore not recommended during breastfeeding due to potential for serious adverse reactions in nursing infants.
