Description
Overview of the medicine
Ceftriaxone is a broad-spectrum cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Usual adult dose: 1-2 gm once daily (every 24 hours) or in two divided doses (every 12 hours) by IV or deep IM injection. In severe infections, the daily dose may be increased to 4 gm.
Elderly
No specific dose adjustment is generally required for elderly patients unless there is severe renal or hepatic impairment.
Renal_impairment
No dose adjustment is needed for mild to moderate renal impairment. For severe renal impairment (creatinine clearance < 10 mL/min), the total daily dose should not exceed 2 gm.
How to Take
For intravenous (IV) administration, reconstitute with appropriate solvent and infuse slowly over 30 minutes. For intramuscular (IM) administration, reconstitute and inject deep into a large muscle mass (e.g., gluteus maximus). Do not exceed 1 gm per single IM injection site.
Mechanism of Action
Ceftriaxone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Rapid, with bactericidal concentrations achieved quickly after administration (within hours).
Excretion
Approximately 50-60% is excreted unchanged in the urine, and 40-50% is excreted in the bile and feces.
Half life
Approximately 6 to 9 hours, allowing for once-daily dosing.
Absorption
Rapid and complete absorption after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are reached within 2-3 hours for IM.
Metabolism
Partially metabolized in the liver to inactive metabolites. About 35-65% is excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to Ceftriaxone, any other cephalosporin, or any component of the formulation.
- •History of severe hypersensitivity reaction to any other type of beta-lactam antibacterial agent (e.g., penicillins, monobactams, carbapenems).
- •Premature neonates up to corrected age of 41 weeks (gestational + chronological age).
- •Term neonates (up to 28 days of age) with hyperbilirubinemia, jaundice, or those requiring calcium-containing IV solutions.
Drug Interactions
Alcohol
May cause a disulfiram-like reaction (flushing, nausea, vomiting, headache) if consumed during or shortly after ceftriaxone treatment, though less common than with some other cephalosporins.
Aminoglycosides
Synergistic antibacterial effect, but should not be mixed in the same syringe or infusion solution due to physical incompatibility.
Calcium-containing solutions
Concomitant use with calcium-containing IV solutions is contraindicated in neonates due to risk of precipitation. In other age groups, administer sequentially with adequate flush between administrations.
Loop diuretics (e.g., Furosemide)
May increase the risk of nephrotoxicity.
Oral anticoagulants (e.g., Warfarin)
May increase the anticoagulant effect and risk of bleeding. Close monitoring of INR/PT is recommended.
Storage
Store dry powder vials below 30°C, protected from light and moisture. Do not freeze. Reconstituted solutions should be used immediately or stored as per specific instructions and discarded if not used within the recommended time.
Overdose
In cases of overdose, symptomatic and supportive treatment should be given. Ceftriaxone is not effectively removed by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown evidence of harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. Ceftriaxone is excreted in low concentrations in breast milk; use with caution during lactation. Observe breastfed infants for diarrhea, candidiasis, or rash.
