Overview of the medicine
Trimax 2 gm Injection contains Ceftriaxone, a broad-spectrum third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
1-2 gm IV/IM once daily or in two divided doses depending on the severity of infection. Max 4 gm/day.
Elderly
Same as adult dosage, but caution with renal/hepatic impairment.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. For severe impairment, monitor plasma concentrations.
How to Take
Administered intravenously (IV) slowly over 30 minutes or intramuscularly (IM) deeply into a large muscle mass.
Mechanism of Action
Ceftriaxone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Onset
Within 30 minutes (IV), 2-3 hours (IM)
Excretion
Approximately 50-60% excreted unchanged in urine, 40-50% excreted via bile and feces.
Half life
Approximately 5.8 to 8.7 hours (prolonged in neonates and elderly)
Absorption
Rapidly and completely absorbed after IM or IV administration.
Metabolism
Partially metabolized in the liver, primarily into inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Ceftriaxone or other cephalosporins
- •Neonates with hyperbilirubinemia
- •Premature neonates
- •Concomitant use with calcium-containing solutions in neonates
Drug Interactions
Loop diuretics
Potentiates nephrotoxicity.
Aminoglycosides
Synergistic antibacterial effect, but increased risk of nephrotoxicity.
Calcium-containing solutions
Fatal precipitation in neonates.
Oral anticoagulants (e.g., Warfarin)
Increased INR/bleeding risk.
Storage
Store below 30°C, protected from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, diarrhea, seizures. Management is primarily supportive and symptomatic. Hemodialysis or peritoneal dialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk in small amounts, use with caution.
