Overview of the medicine
Triocim (Cefixime) 400 mg capsule is a broad-spectrum cephalosporin antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg once daily, or 200 mg every 12 hours. For uncomplicated gonorrhea, a single dose of 400 mg is recommended.
Elderly
No specific dose adjustment is required solely based on age. However, dose should be adjusted based on renal function if impaired.
Renal_impairment
For creatinine clearance 21-60 mL/min, the dose should be reduced to 300 mg daily. For creatinine clearance <20 mL/min, the dose should be 200 mg daily.
How to Take
Triocim capsules should be taken orally with water. It can be taken with or without food. To minimize gastrointestinal upset, it may be taken with food.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell wall disruption and bacterial cell death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-6 hours after oral administration.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. The rest is eliminated via non-renal mechanisms, likely biliary.
Half life
The plasma elimination half-life is typically 3-4 hours, but can be longer in patients with renal impairment.
Absorption
Approximately 40-50% orally absorbed. Absorption may be slightly prolonged by food but not significantly affected.
Metabolism
Cefixime is not extensively metabolized in the body. There are no active metabolites.
Side Effects
Contraindications
- Known hypersensitivity to cefixime, other cephalosporins, or any component of the formulation.
- History of severe hypersensitivity (e.g., anaphylaxis) to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase the absorption and peak plasma levels of cefixime.
Carbamazepine
Concomitant administration may increase plasma levels of carbamazepine. Therapeutic drug monitoring of carbamazepine should be considered.
Oral Contraceptives
May decrease the effectiveness of oral contraceptives, although clinical significance is debated. Additional contraceptive methods may be advised.
Anticoagulants (e.g., Warfarin)
Increased prothrombin time (PT) and international normalized ratio (INR), leading to an increased risk of bleeding. Close monitoring of coagulation parameters is advised.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.

In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by dialysis. Treatment should be symptomatic and supportive.
