Description
Overview of the medicine
Trixim 100 mg Suspension contains Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It works by preventing the growth of bacteria. It is commonly prescribed for respiratory tract infections, urinary tract infections, otitis media, and pharyngitis/tonsillitis.
Uses & Indications
Dosage
Adults
Typically used in children. For adults unable to swallow tablets, dosage is usually 400 mg/day as a single dose or 200 mg every 12 hours.
Elderly
No specific dose adjustment required unless there is severe renal impairment.
Renal_impairment
Creatinine clearance 21-60 mL/min: 75% of standard dose. Creatinine clearance <20 mL/min: 50% of standard dose.
How to Take
Shake the bottle well before each use. Administer orally with a measuring spoon or oral syringe. Can be taken with or without food. Complete the full course of medication even if symptoms improve.
Mechanism of Action
Cefixime, like other beta-lactam antibiotics, inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs) located inside the bacterial cell wall. This prevents the cross-linking of peptidoglycan chains, leading to cell wall defects and eventual bacterial lysis and death.
Pharmacokinetics
Onset
Typically within 1 hour.
Excretion
Primarily renal (approximately 50% unchanged in urine), and some via biliary/fecal route.
Half life
3-4 hours (can be longer in renal impairment).
Absorption
Approximately 40-50% absorbed orally. Food may delay absorption but not significantly affect the extent.
Metabolism
Limited hepatic metabolism; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cefixime or other cephalosporins
- History of severe hypersensitivity reactions to penicillin (cross-allergenicity)
Drug Interactions
Carbamazepine
Elevated carbamazepine levels have been reported when co-administered with cefixime.
Oral anticoagulants (e.g., Warfarin)
Cefixime may increase the anticoagulant effect, leading to increased risk of bleeding. Monitor INR/PT.
Storage
Store dry powder below 30°C, protected from light and moisture. Store reconstituted suspension in a refrigerator (2°C-8°C) and discard after 7-14 days, as directed.
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Excreted in low concentrations into breast milk. Use with caution during lactation.
