Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg once daily or 200 mg every 12 hours. Duration depends on the infection.
Elderly
No specific dosage adjustment is necessary for elderly patients with normal renal function.
Renal_impairment
For creatinine clearance 21-60 mL/min, dose should be 300 mg daily. For creatinine clearance less than 20 mL/min, dose should be 200 mg daily.
How to Take
Trixim 400 mg capsule can be taken orally with or without food. It is recommended to take it at the same time each day to maintain consistent drug levels.
Mechanism of Action
Cefixime is a bactericidal agent that acts by inhibiting bacterial cell wall synthesis. It prevents the cross-linkage of peptidoglycan chains, which is essential for bacterial cell wall integrity.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours post-dose.
Excretion
Primarily renal excretion (glomerular filtration and tubular secretion) of unchanged drug. Also, some biliary excretion.
Half life
Plasma half-life is typically 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food does not significantly alter absorption.
Metabolism
Not extensively metabolized in the liver; about 50% of the absorbed dose is excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase cefixime bioavailability.
Carbamazepine
Concomitant administration may result in increased carbamazepine plasma levels.
Oral contraceptives
May theoretically reduce the efficacy of oral contraceptives, although clinical significance is unclear.
Warfarin and other anticoagulants
May increase prothrombin time (INR), requiring careful monitoring and dose adjustment.

Store below 30°C in a dry place, away from direct sunlight, heat, and moisture. Keep out of reach of children.

In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.

Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Caution is advised when administered to a nursing woman, as it is excreted in human milk.
