Description
Overview of the medicine
Cefixime is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections, including those of the urinary tract, ear, throat, and respiratory system. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Not typically used for adults in suspension form; adults usually take 400 mg once daily or 200 mg every 12 hours as tablet/capsule.
Elderly
No specific dose adjustment is generally required unless there is significant renal impairment.
Renal_impairment
For creatinine clearance < 60 mL/min, dosage adjustment is necessary. For CrCl 21-60 mL/min, total daily dose should be 75% of standard dose. For CrCl < 20 mL/min, total daily dose should be 50% of standard dose.

8 mg/kg/day administered as a single dose or in two divided doses. Max dose 400 mg/day. (e.g., 10-15 kg: 2.5 mL twice daily; 15-20 kg: 3.75 mL twice daily; 20-25 kg: 5 mL twice daily).
How to Take
Administer orally with or without food. Shake the suspension well before each use. Use the provided measuring device (spoon or cup) for accurate dosing.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis, leading to bacterial cell lysis and death.

Onset
Peak plasma concentrations reached within 2-6 hours.
Excretion
Approximately 50% of an absorbed dose is excreted unchanged in the urine over 24 hours. Small amounts are excreted in bile and feces.
Half life
Average plasma half-life of 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food does not significantly alter absorption.
Metabolism
Not extensively metabolized in the body. About 10% may be metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefixime, other cephalosporins, or any component of the formulation.
- Patients with a history of severe hypersensitivity reactions to penicillin (e.g., anaphylaxis).
Drug Interactions
Probenecid
Increases cefixime plasma concentrations by decreasing its renal excretion.
Carbamazepine
Elevated carbamazepine levels have been reported when co-administered with cefixime.
Warfarin and other Anticoagulants
May increase prothrombin time (PT) with or without bleeding. Close monitoring of coagulation parameters is recommended.
Storage
Store the dry powder below 30°C, protected from light and moisture. Keep the reconstituted suspension refrigerated (2-8°C). Do not freeze. Keep out of reach of children.

In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
