Description
Overview of the medicine
Tydin 500 mg Capsule is an oral antibiotic containing Cefixime, a third-generation cephalosporin. It is effective against a wide range of gram-positive and gram-negative bacteria, used to treat various bacterial infections of the respiratory tract, urinary tract, ear, nose, throat, and gonorrhea.
Uses & Indications
Dosage
Adults
The usual adult dose is 200-400 mg daily, administered as a single dose or in two divided doses. For Tydin 500 mg, the typical dose might be 500 mg once daily or as prescribed by a physician based on the specific infection severity and patient condition.
Elderly
No specific dosage adjustment is generally needed for elderly patients unless they have severe renal impairment.
Renal_impairment
For creatinine clearance < 60 mL/min, dosage adjustment is necessary. For creatinine clearance between 21-60 mL/min, the dose should be reduced by 50%. For creatinine clearance < 20 mL/min, the dose should be reduced by 75%.
How to Take
Tydin 500 mg Capsule should be taken orally with or without food. It is crucial to complete the full course of treatment as prescribed by the doctor, even if symptoms improve, to ensure complete eradication of the infection and prevent resistance.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains. This disruption leads to the lysis and death of the bacterial cell.
Pharmacokinetics
Onset
Rapid, with peak plasma concentrations achieved within 2-6 hours after administration.
Excretion
Mainly renal (about 50-60% as unchanged drug) and also through non-renal mechanisms.
Half life
3-4 hours (average).
Absorption
Approximately 40-50% orally absorbed. Food does not significantly affect the rate or extent of absorption.
Metabolism
Minimal hepatic metabolism; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics
- History of serious hypersensitivity reactions (e.g., anaphylaxis) to penicillin or other beta-lactam antibiotics
Drug Interactions
Nifedipine
May increase the bioavailability of cefixime.
Probenecid
Increases cefixime plasma concentrations and prolongs its half-life by decreasing renal tubular secretion.
Carbamazepine
Cefixime may increase carbamazepine plasma concentrations. Monitoring of carbamazepine levels is recommended.
Oral Anticoagulants (e.g., Warfarin)
Concomitant use may increase prothrombin time (INR), requiring careful monitoring of coagulation parameters.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
There is no specific antidote for cefixime overdose. Gastric lavage may be indicated in cases of recent acute overdose. Hemodialysis or peritoneal dialysis is generally not effective in removing cefixime. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use in pregnant women only if clearly needed. Cefixime is excreted in breast milk in small amounts; caution should be exercised when administered to nursing mothers.

Lab Monitoring
- Renal function tests (for prolonged therapy or in patients with pre-existing renal impairment)
- Complete blood count (rarely for prolonged therapy or if blood dyscrasias are suspected)
Doctor Notes
- Emphasize the importance of patient adherence to the full course of therapy to prevent resistance and recurrence.
- Counsel patients on potential gastrointestinal side effects, especially diarrhea, and advise reporting severe or persistent symptoms.
